Purpose: To report a rare case of multiple cactus spines embedded deep in the cornea that were removed with the aid of a fiberoptic illuminator.
Design: Interventional case report.
Methods: A 20-year-old man had an accidental ocular injury of the right eye when he hit a household miniature cactus plant. There were 80 to 90 spiculated foreign bodies embedded in the cornea, some of them even penetrating the anterior chamber.
Results: Removal of the foreign bodies was performed with the aid of a fiberoptic illuminator, which was chosen because it provides superior visualization of the semitranslucent spines. No residual corneal spines were found postoperatively.
Conclusions: Household miniature cactus plants can cause corneal injury, with multiple foreign bodies that can be difficult to localize and remove. The use of a fiberoptic illuminator in such a situation is simple and effective.

Acute suppurative thyroiditis (AST), a rare yet potentially life-threatening infection, comprises less than 1 % of neck pathologies and requires prompt treatment. Symptoms range from neck pain and fever to dysphagia and possible abscess formation. Broad-spectrum antibiotics are the primary treatment; however, surgical drainage may be necessary for abscesses to prevent systemic infection.

Objectives: Cochlear implant array malpositioning is associated with impaired speech perception, vertigo, and facial nerve stimulation. Tip fold-over is a subset of malpositioning that occurs more often with perimodiolar electrodes, but historically it has not been characterized due to lack of knowledge regarding electrode movements of the electrode within the cochlea. Obstetric difficult airway management has emerged as a critical safety issue, and unsuccessful intubation can lead to morbidity and mortality. A literature review of difficult and failed obstetric intubations from the 1970s to 2015 shows that the incidence of failed intubation is unchanged, remaining at one per 390 anesthetics. Our obstetric case report highlights an obstetric difficult airway secondary to limited mouth opening; rescue of the airway with an i-gel®; and establishment of a definitive airway with the aid of an Aintree intubation catheter and flexible fiberoptic scope-guided intubation through the i-gel®, a second-generation supraglottic airway.
