Is D2 lymphadenectomy for gastric cancer a staging tool or a therapeutic intervention?

The extent of lymphadenectomy for gastric cancer is a controversial topic widely debated by those treating the disease. Regional differences in outcome have been noted between patients treated in Japan centers and those treated in Western centers. Technical differences have been investigated within the context of two large, prospective randomized trials, which found no benefit to more extensive lymphadenectomy procedures with increased morbidity. Subsets of patients being treated for cure may benefit from extended resections. The impact of tumor features, such as depth of invasion and number of metastatic lymph nodes, has been described and incorporated into current staging systems. The role of enhanced pathologic evaluation of surgical specimens and impact on staging and treatment strategies is evolving.

P53-abnormal endometrial carcinomas are high-grade and aggressive tumors which should be treated with chemo-/radiotherapy. In low-grade endometrioid carcinoma (LGEC), abnormal expression of p53 is an exceptional finding and is typically accompanied by patchy p16 positivity and diffuse hormone receptor expression. Gastric cancer is a significant health concern worldwide, and its diagnosis and management are of paramount importance. Ectopic pancreas (EP) refers to an embryological abnormality where healthy pancreatic tissue develops without anatomical, vascular, or neural communication with the normal pancreas. We report the case of a patient whose initial endoscopic evaluation suggested early gastric cancer, but computed tomography scan (CT scan) and endoscopic ultrasound (EUS) indicated a locally advanced tumor.

A 62-year-old man was diagnosed with Stage Ⅲ signet ring cell carcinoma of the lower thoracic esophagus. The patient underwent 2 courses of neoadjuvant cisplatin and 5-fluorouracil(SP therapy), demonstrating stable efficacy.
