To certify fitness for (international) air flights one must differentiate between target groups, i.e. pilots, cabin crew and passengers and the stage of infection, i.e. acute and chronic phase of disease and (organ) deficiency after infection, respectively. Publications mostly show indefinite terms which makes principles of quality assessment and international harmonization difficult, in some instances even impossible. Whereas certification of fitness in pilots in respect of infectious diseases should ensure a safe travel problems of infectious diseases in cabin crew and passengers are mainly focused on the risk of avoidable sequelae concerning the person itself and of the transmission to other passengers, respectively.

There is a common perception that peanut/tree nut particles can be transmitted through aircraft ventilation systems and pose a significant risk to passengers with food allergies. In fact, food-induced allergic reactions are around 10-100 times less common during flights than 'on the ground', perhaps because of the multiple precautions food-allergic passengers take when flying. Disruptive passenger behaviour (DPB) incidents spiked during the COVID-19 pandemic period, compromising the safety of commercial flights on a daily basis. This qualitative semi-structured interview study examined the perceived triggering factors and motivations for DPB and the subsequent impact of DPB upon cabin crew well-being and safety. Twenty-four European cabin crew disclosed experiences, subjective observations of perpetrator traits, assessment of DPB development and information regarding their well-being and perceived safety.

In the United States, the Federal Aviation Administration has officially classified flight crews (FC) consisting of commercial pilots, cabin crew, or flight attendants as "radiation workers" since 1994 due to the potential for cosmic ionizing radiation (CIR) exposure at cruising altitudes originating from solar activity and galactic sources. Several epidemiological studies have documented elevated incidence and mortality for several cancers in FC, but it has not yet been possible to establish whether this is attributable to CIR. CIR and its constituents are known to cause a myriad of DNA lesions, which can lead to carcinogenesis unless DNA repair mechanisms remove them.
