Objective: Penile incarceration is an infrequent clinical reality. Diagnosis is evident in most cases, and the challenges are to find the best way to retrieve the constrictive ring and repair the damage. Nevertheless, this event being curious has motivated us to report it.

Methods And Results: We report the case of a 48 year old patient victim of a "joke" resulting in a 13 day penile incarceration produced by 7 double metallic rings, 24 mm in internal diameter and 4 mm wide, which required section and extraction of the rings as well as wide tissue debridement and posterior plastic reconstruction using partial thickness cutaneous grafts. We review diagnostic an therapeutic features from the literature.

Conclusions: Penile incarceration should be considered an emergency, so that the earlier the constrictive object is retrieved the lower the risk for complications secondary to penile devascularization, urinary retention and urethral damage. We emphasise the need to perform retrograde urethrography and suprapubic cystostomy if urethral lesion is suspected, a cutaneous vascular evaluation with doppler ultrasound or fluorescein test and the opportunity of cutaneous grafts to solve tissue lesion.

Penile incarceration with a foreign body is a rare urological emergency necessitating prompt intervention. We present a 59-year-old patient who entrapped penis and right testicle with metal thread reducer for sexual arousal four days prior to admission to Emergency room. Ultrasound revealed significantly reduced Color Doppler signal in right testicle and completely absent signal in penile artery.

Introduction: The rise in the incidence of penile ring incarceration has led to additional cases of ischemic necrosis of the penile skin. Two laterally based scrotal pedicled flaps (LSPFs), noted for their rich blood flow and skin source, resemble penile skin and are the preferred donor sites for repairing such necrosis.

Aims: This study summarizes the outcomes of 2 LSPF grafts for the treatment of penile skin necrosis.

Penile incarceration from constricting metallic or non-metallic objects is a urological emergency which warrants immediate interventions to prevent undesirable sequalae. Various methods such as aspiration technique, string methods, cutting devices and surgery has been described in the literature but each case needs individualized approach due to lack of consensus in the management. We report a case in a 24-year male who incarcerated his penis with a metallic weight plate weighing around 3 kg.

Article Synopsis
  • - Penoscrotal constriction devices can enhance erotic experiences but can lead to emergencies like penile strangulation, requiring urgent medical intervention to prevent lasting damage.
  • - A 47-year-old man with a history of HIV experienced severe pain and swelling after applying stainless steel rings to his penis and scrotum for sexual pleasure, leading to his hospitalization.
  • - Medical staff struggled to remove the constricting rings, ultimately using an electric grinder to cut them, successfully resolving the emergency, making this the first documented case in an HIV-positive patient.
