Avulsion of the triceps tendon during acceleration stress.

Published: September 2002

The authors present a rare case of triceps tendon avulsion associated with +5 G-force and muscle straining. Although failure of musculoskeletal system, mainly the cervical spine, secondary to high-G exposure has been reported, this is the first report they have found of a triceps tendon avulsion. Furthermore, unlike other traumatic triceps avulsions reported, the pilot involved had no known predisposing factors. Awareness of the possibility of such an injury must be maintained when a pilot of high performance aircraft presents with elbow pain.

Background: Distal triceps tendon injuries are relatively rare injuries, often occurring in highly active patients with physically demanding jobs or lifestyles. Information on return to work, sport, and activity is essential for patient education and counseling after a distal triceps tendon rupture.

Purpose: To determine the rates of return to work, sport, and sport-related activity after distal triceps tendon repair.

Avulsion of the triceps tendon is a rare injury accounting for less than 1% of all tendon injuries. The triceps is an extensor of the elbow and causes compromised function if left untreated. Complete ruptures should be treated with early repairs for satisfactory outcomes.

This retrospective study is the only one in the last 10 years from central Europe and provides a current picture of prevalence, new diagnostic modalities, new methods of surgical treatment, and also offers new insights into post-operative care. Triceps tendon rupture is the least reported among all the tendon injuries in the literature. In general, effective treatments for tendon injuries are lacking because the understanding of tendon biology lags behind that of the other components of the musculoskeletal system.

Persisting deficits are often seen years after an Achilles tendon rupture despite dedicated rehabilitation efforts. A possible reason for reduced function is elongation of the tendon and accompanying shortening of the muscle. Strength training with focus on the eccentric component of loading leads to longer muscle fascicles in healthy persons.

A high proportion of individuals with Achilles tendinopathy continue to demonstrate long-term symptoms and functional impairments after exercise treatment. Thus, there is a need to delineate patient presentations that may require alternative treatment. The objective of this study was to evaluate if the presence of metabolic risk factors relates to tendon symptoms, psychological factors, triceps surae structure, and lower limb function in individuals with Achilles tendinopathy.
