Background: Primary extramedullary plasmacytomas (PEMP) are rare malignant neoplasms with a predilection for the head and neck. Eighty percent of all PEMP are located in this area.

Angiosarcoma is a rare soft tissue sarcoma, namely when it presents as a primary intradural extramedullary spinal neoplasm, with only one case of non-vertebral origin reported in the literature. We present the case of a 51-year-old woman with neurological symptoms of paraparesis and constipation who underwent a magnetic resonance imaging (MRI) that revealed a well-demarcated, predominantly homogeneous, intensely enhancing intradural extramedullary lesion in T2-weighted images. Histology, immunohistochemistry, and genetics of the lesion showed an angiosarcoma.

Rationale: Extramedullary plasmacytoma is an extremely rare malignant clonal plasmacytoma, which can occur in any tissue or organ of the body other than bone marrow hematopoietic tissue. Lesions mostly occur in the head and neck or upper respiratory tract, and rarely in the lower respiratory tract. Primary plasmacytoma of the lung, also known as primary pulmonary plasmacytoma (PPP), is rarer and is mainly diagnosed on the basis of the histopathology of biopsy tissue.

Hematopoiesis is a process by which all blood cells are formed. The mechanisms controlling it have been studied for decades. Surprisingly, while hematopoietic stem cells are among the most extensively studied stem cell types, the complete understanding of how they are regulated during development, adulthood, or in non-homeostatic conditions remains elusive.
