Postictal asystole during ECT. | LitMetric

Postictal asystole during ECT.

J ECT

Department of Psychiatry, Southern Illinois University, Springfield, Illinois 62794-9642, USA.

Published: June 2002

Background: Asystole is an uncommon but potentially fatal complication of electroconvulsive therapy (ECT). Several risks for poststimulus asystole have been described, but risks for asystole at other times have not.

Method: Two instances of ECT postictal asystole in healthy adult males are reported. Features in common are identified.

Results: features shared differ from risk factors for poststimulus asystole. In common were adult but not geriatric age, male sex, good physical and cardiac health, mesomorphic habitus, anticholinergic pretreatment, vigorous ECT seizure, and low resting heart rate. Both patients showed postictal bradyarrhythmia at the previous ECT.

Conclusions: Postictal asystole has apparent similarities to postexertional asystole in athletes. The combination of higher pretreatment doses of an atropinic agent, a sympatholytic agent, and close monitoring of postictal cardiac rhythm should be considered with patients similar to ours, especially after occurrence of postictal bradyarrhythmia.

Update review on SUDEP: Risk assessment, background & seizure detection devices.

Epilepsy Behav

November 2024

Academic Center for Epileptology Kempenhaeghe, Heeze, The Netherlands; Eindhoven University of Technology, Eindhoven, The Netherlands; Department of Neurology, Ghent University Hospital, Ghent, Belgium.

Article Synopsis
  • The review examines sudden unexpected death in epilepsy (SUDEP), focusing on risk factors, scoring systems to assess risk, and mechanisms behind SUDEP occurrences.
  • Key risk factors include the presence and frequency of generalized tonic-clonic seizures (GTC), with odds ratios indicating significant risk, particularly in specific contexts like bedroom sharing.
  • Four main SUDEP risk scoring systems are discussed, highlighting their different designs and the need for further validation in general epilepsy populations, along with proposed mechanisms of SUDEP involving brainstem dysfunction and potential roles of adenosine and serotonin.

Respiratory activity during seizures induced by pentylenetetrazole.

Respir Physiol Neurobiol

May 2024

Department of Physiology and Biophysics, Institute of Biological Science, Federal University of Minas Gerais (ICB/UFMG), Belo Horizonte, Brazil. Electronic address:

This study investigated the respiratory activity in adult Wistar rats across different behavioral seizure severity induced by pentylenetetrazole (PTZ). Animals underwent surgery for electrodes implantation, allowing simultaneous EEG and diaphragm EMG (DIA) recordings and the respiratory frequency and DIA amplitude were measured. Seizures were acutely induced through PTZ injection and classified based on a pre-established score, with absence-like seizures (spike wave discharge (SWD) events on EEG) representing the lowest score.

The Heart and Seizures: Friends or Enemies?

J Clin Med

September 2023

IRCCS Istituto delle Scienze Neurologiche di Bologna, Unit of Neurology, Bellaria Hospital, 40139 Bologna, Italy.

The heart and seizures are closely linked by an indissoluble relationship that finds its basis in the cerebral limbic circuit whose mechanisms remain largely obscure. The differential diagnosis between seizures and syncopes has always been a cornerstone of the collaboration between cardiologists and neurologists and is renewed as a field of great interest for multidisciplinary collaboration in the era of the diffusion of prolonged telemonitoring units. The occurrence of ictal or post-ictal arrhythmias is currently a cause of great scientific debate with respect to the role and risks that these complications can generate (including sudden unexpected death in epilepsy).
