The surgical treatment of burst fracture, tumour or spondylitis remains a challenge with regard to the surgical approach to the anterior aspect of the cervicothoracic junction. Many vital structures including osseus, articular, vascular and nervous ones hinder the exposure. Fortunately, indications for surgery in this region are rare. However, when it becomes necessary for the surgeon to expose this region, it is useful to be prepared to approach it carefully. In this investigation the anatomy and exposure of the cervicothoracic junction by means of a sternotomy are described. An illustrated review of the sternotomy approach to the cervicothoracic junction with a description of 'tricks and pitfalls' is provided.

The combination of thoracoscopic and transmanubrial approaches provides good visualization and maneuverability of the cervicothoracic junction area. Here we report the case of a young woman with a mediastinal tumor extending to the neck who first underwent a thoracoscopic approach followed by a transmanubrial approach. This procedure was superior in terms of technical advantages and preserved function as well as minimally invasive and cosmetically pleasing.

Background: Previous research on spinal alignment and postoperative outcomes after cervical and upper thoracic fixation has suggested that clinical and patient-reported outcomes are improved when certain anatomical parameters are maintained. These parameters include the cervical sagittal vertical axis (cSVA), C2 and T1 slopes, and cervical lordosis (CL). Ga-DOTATOC PET/CT is a functional imaging modality that has revolutionized the evaluation of well-differentiated neuroendocrine tumors (NETs) by targeting somatostatin receptors. This technique has largely replaced conventional gamma camera imaging with 111In-labeled octreotide due to its superior sensitivity and resolution.
