Surgery of the Primary Tumor in de novo Metastatic Breast Cancer: A Palliative Approach or a Potential Survival Game-Changer?

Ann Ital Chir

March 2025

Department of Woman and Child Health and Public Health, Fondazione Policlinico Universitario Agostino Gemelli IRCCS, Università Cattolica del Sacro Cuore, 00168 Rome, Italy.

Approximately 5-10% of primary breast cancer cases present as de novo stage IV disease, characterized by distant metastases at diagnosis. Traditionally, systemic therapies such as chemotherapy, endocrine therapy and targeted treatments have formed the cornerstone of treatment for metastatic breast cancer (MBC), focusing on disease control, symptom palliation and quality of life improvement. While systemic therapies remain crucial, the role of local treatments, particularly surgery for the primary tumor, is increasingly debated.

Minoritized groups experience interpersonal, structural, and systemic marginalization that is also perpetuated within academic institutions. This marginalization produces barriers that exclude racial/ethnic minoritized groups within academic medicine from career opportunities and advancement. Racial/ethnic minoritized faculty are often expected to take on additional labor to serve the diversity needs of the program and/or institution that are often unrecognized or undervalued in the tenure or promotion process or detract from additional responsibilities.

Introduction: Mobile mammography (MM) can reach women who otherwise would not or could not utilize facility-based mammography (FBM) due to various socioeconomic factors and access barriers. Most literature evaluates individual MM programs and has not explored the potential impact of MM in a large, nationally representative, claims dataset.

Objective: To examine factors associated with MM utilization among female Medicare beneficiaries and whether MM is associated with increased mammography utilization compared use of FBM only.

Principles and Paradigms of De-Escalated Elective Nodal Irradiation: Boldly Going Towards an Inflection Point in Head and Neck Radiotherapy.

Semin Radiat Oncol

April 2025

Department of Radiation Oncology, University of Toronto, Toronto, Ontario, Canada.; Radiation Medicine Program, Princess Margaret Cancer Centre, University Health Network, Toronto, Ontario, Canada.. Electronic address:

Fundamental axioms of elective nodal irradiation (ENI) for head and neck cancers merit re-examination in contemporary practice. Standard ENI doses to volumes bordering critical organs-at-risk increased during the transition from two-dimensional radiation planning to intensity-modulated radiotherapy, despite improvements in detection of occult nodal metastases with modern imaging, use of concurrent chemotherapy, and identification of human papillomavirus (HPV)-related radiosensitivity. Historical large ENI volumes covering low-risk nodal regions continue to be commonly used even as awareness grows regarding the predominant pattern-of-failure within existing gross disease.

Purpose: This study aims to evaluate the predictive value of MRI features for pathologic complete response (pCR) and survival outcomes in patients with breast cancer (BC) undergoing neoadjuvant chemotherapy (NAC).

Methods: A retrospective analysis was conducted on 168 BC patients treated with NAC between 2018 and 2022. Pre-NAC breast MRI scans were evaluated for enhancement patterns, time-intensity curve (TIC), peritumoral edema, and background enhancement.
