Predictors of nephrological morbidity after coronary artery bypass surgery.

Cardiovasc Surg

Department of Thoracic and Cardiac Surgery -414-, University Hospital, Nijmegen, The Netherlands.

Published: June 2002

Aim: The aim of this study was to analyse the nephrological morbidity after myocardial revascularization.

Methods And Results: We analysed the pre- per and postoperative data of 3815 patients who underwent a primary isolated coronary bypass grafting between January 1987 and December 1995. Nephrological complications were divided into renal dysfunction and requiring dialysis. The increase in nephrological complications (1.2-4%) is the result of an increase of patients with renal dysfunction. Unifactorial risk analysis identified age, diabetes, hypertension, nephrological pathology, perfusion time, aortic cross-clamp time, emergency operation and perioperative myocardial infarction as risk factors. Multifactorial risk analysis identified age, diabetes, hypertension, nephrological pathology, perfusion time, perioperative myocardial infarction and the cohorts operated upon in 1990-92, 1993-95 as independent risk factors for renal dysfunction, and age, nephrological pathology and perioperative myocardial infarction for those needing dialysis.

Conclusion: Several variables have been identified as risk factors for nephrological complications; especially important are pre-existing nephrological pathology and perioperative myocardial infarction.

Secondary hyperparathyroidism (sHPT) is a significant clinical complication of CKD leading to bone abnormalities and cardiovascular disease. Current treatment based on activating the parathyroid calcium-sensing receptor (CaSR) using calcimimetics such as Cinacalcet, aims to decrease plasma PTH levels and inhibit the progression of parathyroid hyperplasia. In the present study, we found significant diurnal rhythmicity of Casr, encoding the Cinacalcet drug target in hyperplastic parathyroid glands (p = 0.

A case of enamel renal syndrome from a novel genetic mutation, multidisciplinary management and long-term prognosis.

Ups J Med Sci

October 2024

Institute of Surgical Sciences, Department of Plastic and Oral & Maxillofacial Surgery, Uppsala University, Uppsala, Sweden.

Article Synopsis
  • Enamel renal syndrome (ERS) presents unique challenges in diagnosis and treatment, primarily involving issues with enamel formation and kidney health, making patient management complex and often requiring specialized dental care.
  • A case study demonstrates the long-term management of a patient diagnosed with ERS after a delay in diagnosis, including genetic confirmation and ongoing nephrological monitoring for related complications.
  • The findings emphasize the necessity for integrated dental and medical care for patients with syndromic disorders, as well as the need for continuous monitoring of renal health, which shows a favorable prognosis despite the intricate nature of the condition.

Article Synopsis
  • Hemolytic uremic syndrome (HUS) is a serious condition that can happen after an infection or due to problems in the body's immune system, and there are two types: typical and atypical.
  • This text discusses a 9-year-old boy who had a rare form of aHUS caused by specific genetic issues, which made his body produce autoantibodies that affected his kidneys and brain.
  • The study found that many kids with aHUS needed kidney treatment and that early treatment with special therapy may help prevent kidney failure, even if the patient has infections like STEC.

Department of Internal Medicine, Centre de référence des maladies autoinflammatoires et des amyloses (CEREMAIA), DMU i3d, AP-HP, Hôpital Tenon, Service de Médecine Interne, Sorbonne University, Tenon Hospital, 75020, Paris, France.

Article Synopsis
  • AA amyloidosis (AAA) is a serious multisystem disease stemming from chronic inflammation, commonly affecting the kidneys and posing risks during pregnancy.
  • A study reviewed cases of AAA in pregnant women, identifying three new patients and highlighting that familial Mediterranean fever was the leading cause of AAA in this group.
  • The research found that pregnancies in AAA patients often led to complications like growth retardation and preterm birth, indicating the need for heightened medical monitoring during pregnancy.
