The occipitocervical junction is formed by the foramen magnum (FM) and the adjacent anatomical structures of the C1 and C2 vertebrae. The FM is formed anteriorly by the basilar part of occipital bone. Anterolaterally, it borders the occipital condyles and hypoglossal canal as well as the jugular foramen.

Aims and objectives This study aimed to analyze the mandibular foramen (MF) by identifying its average vertical and horizontal diameters, determining its localization, and assessing the prevalence of accessory mandibular foramina. Methodology The study was conducted at the Institute of Anatomy, Madras Medical College, Chennai, India over one month in August 2024. Background: We describe techniques for safe resection of a Type 3 foramen magnum meningioma with dorsal displacement of the accessory nerve rootlets and vertebral artery which limits ventral access to the tumor.

Method: Partial sectioning of the accessory nerve rootlets may help create larger working space. Topical lidocaine placement on the rootlets of the spinal accessory nerve may mitigate trapezius muscle contraction and facilitates further progress throughout tumor resection.
