Video-assisted thoracoscopic surgery is emerging as a viable approach to increasingly complex intrathoracic therapeutic procedures. Here, we present a case of eventrated left hemidiaphragm caused by a blunt trauma in an elderly man. The diaphragm was repaired successfully using a video-assisted procedure, thus giving the patient the advantages of a minimally invasive operation.

The patient was a 66-year-old man who developed exacerbation of respiratory and heart failure that necessitated tracheal intubation. The exacerbation was considered to be largely caused by asthma-chronic obstructive pulmonary disease overlap syndrome and type 3 pulmonary hypertension. However, left diaphragmatic eventration and tracheal stenosis were also found.

Diaphragmatic eventration (DE) is characterized by abnormal diaphragm elevation resulting from muscle weakness or thinning. It may be congenital or acquired, affecting both pediatric and adult populations, and is associated with symptoms such as dyspnea and chest pain. Although DE is infrequent, with a higher incidence on the left side, it has traditionally been managed through invasive surgical techniques.
