Fibrosis is one of the hallmarks of inflammatory and repair processes in pathology. Various exogenous and endogenous stimuli, including tumor development, can induce inflammatory reactions. During the post-equilibrium phase after IV injection of non specific contrast media, CT and/or MR allow the study of these inflammatory answers to tumoral or infectious processes. Delayed enhancement of collagenic fibrous tissue during the late post-equilibrium phase is an essential complementary data in the characterization of many liver lesions: cirrhosis, cholangiocarcinoma, focal nodular hyperplasia, fibrous metastasis. but also for the differential diagnosis of pancreatic diseases (groove pancreatitis vs ductal adenocarcinoma) or of gastro-intestinal diseases (gastric adenocarcinoma vs lymphoma, mechanical complication vs inflammatory bouts of ileal Crohn's disease).

[Groove pancreatitis. Diagnostic impact of dynamic radiology].

Introduction: Described by Becker in 1973, groove pancreatitis is a rare segmental chronic pancreatitis affecting the groove between the head of the pancreas, the duodenum and the common bile duct. Mimicking pancreatic head carcinoma, surgical exploration is often required.

Observation: Diagnosis of groove pancreatitis was made on computed tomography and MR imaging (groove fibrosis) in a 58 year-old man presenting with acute pancreatitis complicated by bilary and duodenal stenosis, with totally favourable outcome within 5 months with conservative treatment.

Purpose: To determine the usefulness of delayed post-equilibrium-phase contrast material-enhanced images in evaluation of intrahepatic cholangiocarcinoma.

Materials And Methods: Review of surgical, pathologic, and radiologic records revealed 47 patients with proved cholangiocarcinoma with delayed (6-36 minutes) post-equilibrium-phase contrast-enhanced computed tomographic (CT) scans. Unenhanced, dynamic contrast-enhanced, and delayed images were retrospectively reviewed, and enhancement patterns for cholangiocarcinoma lesions were recorded.
