Elevation of the malar fat pad with a percutaneous technique.

Objective: To describe a simplified method of malar fat pad elevation to rejuvenate the midface, nasolabial folds, labiomental folds, infraorbital hollows, and jowls via a percutaneous approach.

Patients And Design: One hundred eighteen patients were evaluated over a 12-month period in a prospective fashion by the operating surgeon. Preoperative and postoperative photographs were used for comparison.

Setting: Private practice.

Main Outcome Measures: Elevation of the malar fat pad by a measured amount.

Results: At 3 months, all patients had a significant elevation of the malar fat pad (3-7 mm), with the exception of 2 early patients who underwent revision surgery, with satisfactory results.

Conclusions: A simplified method of malar fat pad elevation is described. This method allows for elevation of the malar fat pad without extensive dissection. Excellent results are obtained with very little morbidity.

Background: In most of the published plication techniques in face lift surgery, the vectors of plication are not entirely superiorly and vertically directed. The same applies with the deep plane, SMAS elevation techniques in the majority of which the vectors of traction are not superiorly vertically directed. The aging symptoms are mostly prominent at the anterior mobile face due to the gravity effect, and this is the area where attention should be focused to correct these symptoms following a face lift surgery.

Background: This investigation assessed patient satisfaction with a temporal incision technique using a super-high superficial musculoaponeurotic system (SMAS) flap for midface lifting in a Chinese patient cohort.

Methods: From July 2019 to July 2023, 95 patients underwent midface lifts via a temporal incision approach at our institution. The extent of SMAS flap dissection spanned 2 cm above the outer canthus, to the lower margin of the zygomatic arch inferiorly, and medially to the zygomaticus major muscle.
