A hard deep nodule, 6 x 3 mm in size, was excised from the cheek of a 12-year-old girl. The histological sections showed a multinodular cyst-like proliferation, each nodule showing the typical structure and cytology of pilomatrixoma. A comparison is made between this case and the budding phenomenon that has been described in isthmus-catagen (tricholemmal) cyst.

An 80-year-old female presented with a slowly growing 2-cm nodule on her shoulder over a 1-year period. Histopathologic sections of a biopsy specimen showed a multinodular, dermal-based basaloid tumor with areas of clear-cell change, stromal induction, as well as significant cytologic atypia and atypical mitotic activity. An initial investigation revealed positive staining of CDX2, a well-known marker of tumors of gastrointestinal origin.

DICER1, a RNAse endonuclease involved in the processing of siRNA and microRNA, is known to play a pivotal role in the post-transcriptional regulation of gene expression. Germ line mutations in the DICER1 gene increase the risk for different types of tumors. At present, DICER1 syndrome is an established, though not well defined, member of the group of genetic tumor predisposition syndromes.

Pilomatricoma (PM; calcifying epithelioma of Malherbe) is a benign tumor originating from the matrix of the hair follicles. Clinical types of the tumor are familial, perforating, multinodular, exophytic, anetodermic, and giant PM. The bullous type is seen only in 3 to 6% of cases.

We report a particularly uncommon location of pilomatrixoma: the scrotal skin. A 46-year-old man, without history, has developed a scrotal, multinodular firm tumor, measuring 10 cm.
