Intravenous administration of 400-600 mg/day potassium canrenoate normalised plasma osmolarity and permitted rapid re-establishment of the metabolic picture in 9/12 patients with hyperosmolarity admitted to a resuscitation department for complex physiopathological pictures. The soundness of the drug in this sense is affirmed, as well as its permission of the recommencement of parenteral nutrition.

Department of Endocrinology, Shanghai Pudong Hospital, Fudan University, Shanghai, 201399, People's Republic of China.

Article Synopsis
  • * Researchers analyzed data from 565 diabetic patients and found significantly higher levels of carbohydrate antigen 199 (CA199) in DKA and carcinoembryonic antigen (CEA) in HHS, alongside other clinical laboratory findings indicating altered metabolic and organ function.
  • * The results suggest that elevated CA199 in DKA is linked to acidosis, while elevated CEA in HHS may relate to iron balance, with osmolarity identified as a key

Article Synopsis
  • The study investigates the effectiveness and safety of hypertonic saline (HTS) compared to mannitol in reducing intracranial pressure (ICP) during brain tumor craniotomy in adults.
  • A systematic review of eleven randomized controlled trials (RCTs) with 593 participants showed that HTS resulted in greater brain relaxation and lower urine output compared to mannitol, while also improving mean arterial pressure.
  • While HTS was found to be superior in achieving brain relaxation, it did lead to higher plasma sodium levels, highlighting a trade-off in its usage during surgery.

Hereditary spherocytosis (HS) is a membranopathy that impacts the vertical junctions between the cytoskeleton and the plasma membrane of erythrocytes. The gold standard method for diagnosing it is osmotic gradient ektacytometry (OGE). However, access to this technique is scarce.

Article Synopsis
  • The study examines the use of renal replacement therapy (RRT) to treat hyperammonemia in children, particularly focusing on its impact on plasma osmolarity and the risk of neurological issues.
  • A total of 11 pediatric patients were analyzed, revealing a significant correlation between high ammonia levels and increased plasma osmolarity, with potential complications such as brain herniation occurring during rapid osmolar changes.
  • The findings emphasize the importance of monitoring plasma osmolarity in young patients undergoing RRT to prevent serious neurological complications.
