A 68-year-old male presented with abdominal pain and obstructive jaundice. CT revealed a large mass in the pancreatic head that was initially interpreted as pancreatic carcinoma. Needle biopsy revealed only fibrous tissue with signs of chronic inflammation. Together with typical findings of an idiopathic retroperitoneal fibrosis, the final diagnosis of multifocal idiopathic fibrosclerosis with focal pseudotumorous pancreatic head fibrosis could be made.

Objectives: Naldemedine is a peripherally acting μ-opioid receptor antagonist used to treat opioid-induced constipation. As this drug does not cross the blood-brain barrier, it is believed that patients without brain metastases do not experience opioid withdrawal symptoms.
Methods: Here, we experienced a case in which a cancer patient without brain metastasis presented with anxiety and restlessness that was severe enough to interfere with daily life.

A 50-year-old female presented with abdominal pain. Upper gastrointestinal endoscopy revealed a 30 mm ulcerative lesion extending from the duodenal bulb to the descending portion, and biopsy confirmed poorly differentiated adenocarcinoma. Abdominal contrast-enhanced CT scan showed an hypovascular tumor in the pancreatic head with suspected invasion into the duodenum, along with enlarged #8 lymph node.
