Mycobacterium kansasii most commonly causes a slowly progressive pulmonary disease. Skin and disseminated infections are seen less frequently and only in immunocompromised hosts. To our knowledge, no case of Mycobacterium kansasii infection or skin infection associated with additional organ involvement in an immunocompetent patient has been reported.

Mycobacteria infections are caused by species of the Mycobacterium tuberculosis complex (MTB) and other species called Non-Tuberculosis Mycobacteria (NTM). Identification of mycobacteria species is very important to define treatment and it can be achieved by direct culture. However, the lack of clear protocols regarding the use of culture or molecular tests on specimens diagnosed with granulomatous lesions causes delays in the diagnosis of the etiological agents and, consequently, the definition of the right treatment.

Nontuberculous mycobacteria (NTM) are a diverse group of environmental mycobacteria found ubiquitously in soil and water, causing infections that typically arise from environmental exposure rather than person-to-person transmission. This case report documents the first known detection of in our region, emphasizing the clinical challenges and diagnostic complexities associated with NTM infections. The case involves a 37-year-old male with a significant smoking history who presented with progressively worsening cough, significant weight loss, and bilateral cavitary lung lesions.
