Trihexyphenidyl (Artane) is a centrally active muscarinic antagonist commonly used to treat patients with generalized dystonia. In a retrospective survey of 22 consecutive children with extrapyramidal cerebral palsy, we evaluated trihexyphenidyl on upper extremity and lower extremity function, expressive language, and drooling. Functional changes were assessed using a parental questionnaire (rating scale 1-5: from 1 = little or no change to 5 = tremendous change, with scores in either a positive or negative direction). Improvements of +4 or +5 were reported in eight children for upper extremity function, in eight children for verbal expressive language, in five for drooling, and in none for lower extremity function. Using bivariate linear regression modeling to investigate variables associated with treatment effects, there was a significant inverse relationship between age at initiation of medication and therapeutic response. Furthermore, beneficial responses were specific to upper-extremity function and expressive language. These results suggest that younger children are more likely to respond to trihexyphenidyl and that primary functional benefits include improved fine motor abilities and expressive language. A prospective masked study with a standardized clinical instrument is needed to confirm these findings.

Background: This study examined the profiles of adaptive behavior development in adults with autism spectrum disorder (ASD) and severe intellectual disability (ID), and the relationships between the levels of the different domains and subdomains of adaptive development and the intensity of autistic symptomatology.
Participants: This study involved 71 adults (44 men and 27 women with average ages of 39 years 7 months and 36 years 2 months, respectively) living in medico-social institutions and having a level of adaptive development corresponding to age below 3 years 4 months and a level of cognitive development corresponding to ages between 12 and 24 months.
Methods: ASD was diagnosed using Pervasive Development Disorder-Mental Retardation Scale (PDD-MRS) and Childhood Autistic Rating Scale (CARS), ID and its severity were determined based on the Diagnostic Statistical Manual-5 (DSM-5) criteria, and the very low cognitive developmental level was assessed using the Socio-emotional Cognitive Evaluation Battery (Adrien, Pearson-ECPA, 2007), adapted for adults (SCEB-A).

Purpose: Toddlers with Down syndrome (DS) showcase comparable or higher rates of gestures than chronological age- and language-matched toddlers without DS. Little is known about how gesture use in toddlers with DS relates to multiple domains of development, including motor, pragmatics, language, and visual reception (VR) skills. Developmental Language Disorder (DLD) is a clinical condition characterised by language difficulties without cognitive or neurological impairments, leading to communication and learning challenges. The study explores the narrative and linguistic abilities of children with DLD and Typically Developing (TD) peers by analysing both macrostructural and microstructural aspects of their narrative production elicited during a storytelling task and describing the types of grammatical and lexical errors. Background: Cerebral creatine deficiency disorders (CCDD) are rare diseases caused by defects in the enzymes L-arginine: glycine amidinotransferase (AGAT) or guanidinoacetate-N-methyltransferase (GAMT), which are involved in synthesis of creatine; or by a defect in the creatine transporter (CRTR), which is essential for uptake of creatine as important energy source into the target cells. Patients with CCDD can present with a variety of unspecific symptoms: global developmental delay, speech-language disorder, behavioral abnormalities and seizures. Early treatment initiation is essential in AGAT and GAMT deficiencies to achieve a favorable outcome.
