Image-Guided Selective Nerve Blocks in the Spine.

Selective nerve block (SNB) in the spine is a diagnostic and therapeutic imaging procedure. It consists of placing a needle under fluoroscopic guidance in the epiradicular space, which is formed by extensions of the anterior and posterior epidural membranes. Injection of nonionic contrast will confirm the correct position of the needle. This is followed by injection of a long-acting local anesthetic and/or corticosteroid. Pain reproduction followed by pain relief is recorded. This technique enables one to identify a symptomatic nerve, produce pain relief for a variable period of time, and help select surgical candidates.

This study aimed to compare the anesthetic efficacy of 2% lignocaine with a 20-gauge needle in an inferior alveolar nerve block (IANB) and 4% articaine with a 24-gauge needle in a buccal nerve block (BNB) during the extraction of dentoalveolar abscesses in children aged 5-11 years.

Direct repair of ulnar collateral ligament (UCL) injuries with suture augmentation has been successful in properly selected patients lacking chronic attritional wear of the medial elbow. Described is a Speed-Fix technique for direct UCL repair using SutureTape, with Brace augmentation. The Speed-Fix repair technique uses an inverted mattress knotless repair with a knotless SwiveLock anchor and FiberTape suture, which allows for theoretical compression at the repair site.

Objectives: The current gold standard for immunofluorescent (IF) visualization of neuromuscular junctions (NMJs) in muscle utilizes frozen tissue sections with fluorescent conjugated antibodies to demarcate neurons and IF alpha-bungarotoxin (α-BTX) to demarcate motor endplates. Frozen tissue sectioning comes with inherent inescapable limitations, including cryosectioning artifact and limited sample shelf-life. However, a parallel approach to identify NMJs in paraffin-embedded tissue sections has not been previously described.
