Real-time radiology--new concepts for teleradiology.

We propose two new concepts for teleradiology: 'real-time radiology' and 'seamless operation of a workstation to view images from both local and remote hospitals'. In this paper, we describe an experimental real-time radiology system with a new multimedia diagnostic workstation, which has been established under teleradiology concepts and can be used for cooperative diagnosis by interchanging radiographic images bilaterally in real-time during an audio-visual discussion. The clinical evaluation of real-time radiology using a newly developed diagnostic workstation demonstrated the system to be practical and routinely available to enhance the radiological diagnostic quality of teleradiology.

Purpose: The AAST Organ Injury Scale is widely adopted for splenic injury severity but suffers from only moderate inter-rater agreement. This work assesses SpleenPro, a prototype interactive explainable artificial intelligence/machine learning (AI/ML) diagnostic aid to support AAST grading, for effects on radiologist dwell time, agreement, clinical utility, and user acceptance.

Methods: Two trauma radiology ad hoc expert panelists independently performed timed AAST grading on 76 admission CT studies with blunt splenic injury, first without AI/ML assistance, and after a 2-month washout period and randomization, with AI/ML assistance.

The radiologists were traditionally working in the background. What upgraded them as physicians during the second half of the past century was their clinical training and function precipitated by the evolution of Interventional Radiology and Medical Imaging, especially with ultrasonography.
