Objectives: To determine the effects of sustained, 3-day endotoxin infusion on early steps of the insulin-signaling pathway in rat liver and skeletal muscle in vivo; to examine insulin signaling in well-established acute endotoxin models of insulin resistance.

Design: Prospective, controlled animal study.

Setting: University research laboratory.

Subjects: Male Sprague-Dawley rats: 24 in the 3-day endotoxin study, 22 in each acute endotoxin study.

Interventions: In prolonged endotoxemia studies, endotoxin (1 mg.kg-1.24 hrs-1) was administered via jugular venous catheter for 74 hrs. Insulin was then injected, and liver and skeletal muscle were removed after 5 mins. In acute endotoxemia studies, an endotoxin bolus (1 mg/kg) was administered, and insulin-signaling responses were studied after 4 hrs.

Measurements And Main Results: In liver of rats with sustained endotoxemia, there were significant decreases in insulin-stimulated tyrosine phosphorylation of insulin receptors (74%), insulin receptor substrate (IRS)-1 (74%), and IRS2 (53%); binding of the p85 subunit of phosphatidylinositide 3-kinase to IRS1 (80%); and IRS1-precipitable phosphatidylinositide 3-kinase activity (>90%). These findings were associated with significant reductions in abundance of insulin receptors (37%), IRS1 (60%), and IRS2 (23%). Signaling in skeletal muscle was similarly affected, with reduced IRS1 phosphorylation (49%), IRS1 abundance (50%), and binding of p85 to IRS1 (57%). Insulin signaling 4 hrs after endotoxin administration was not different from controls.

Conclusions: Prolonged endotoxemia is associated with marked deficits in early steps of the insulin-signaling pathway, which are at least partly explained by reduced abundance of the insulin receptor and IRS proteins. Signaling defects were not evident 4 hrs after endotoxin administration under conditions of adequate nutrition, indicating that insulin resistance develops gradually, may require concomitant malnutrition, and is not reversed by the development of endotoxin tolerance.

Purpose: To explore the evaluation value of contrast enhanced ultrasound (CEUS) quantitative parameters in ischemic-type biliary lesions after liver transplantation to assist its early-diagnosis.

Methods: Patients who underwent liver transplantation and intravenous CEUS at Beijing Friendship Hospital, Capital Medical University from June 25, 2020 to December 28, 2022 and were diagnosed with Ischemic-type biliary lesions (ITBLs) by Magnetic Resonance Cholangiopancreatography (MRCP) or Endoscopic Retrograde Cholangiopancreatography (ERCP) or Percutaneous Transhepatic Cholangiography (PTC) were prospectively enrolled. SonoLiver software was used to quantitatively analyze the contrast images, transplanted livers with normal biliary tracts as the control group.

The burdens of cardiovascular (CV) diseases and cardiotoxic side effects of cancer treatment in oncology patients are increasing in parallel. The European Society of Cardiology (ESC) 2022 Cardio-Oncology guidelines recommend the use of standardized risk stratification tools to determine the risk of cardiotoxicity associated with different anticancer treatment modalities and the severity of their complications. The use of the Heart Failure Association-International Cardio-Oncology Society (HFA-ICOS) is essential for assessing risk prior to starting cancer treatment, and validation of these methods has been performed in patients receiving anthracyclines, human epidermal receptor 2 (HER2)-targeted therapies and breakpoint cluster region-abelson oncogene locus (BCR-ABL) inhibitors.

Introduction: Gait analysis is a vital tool in the assessment of human movement and has been widely used in clinical settings to identify potential abnormalities in individuals. However, there is a lack of consensus on the normative values for gait metrics in large populations. The primary objective of this study is to establish a normative database of spatiotemporal gait metrics across various age groups, contributing to a broader understanding of human gait dynamics.

Laparoscopic cholecystectomy for acute cholecystitis carries an increased risk of biliovascular injuries. Fluorescence cholangiography (FC) is a valuable diagnostic tool for identifying extrahepatic bile ducts (EHBD). The objective of this study was to evaluate the efficacy of FC in delineating EHBD anatomy, both before and after dissection, based on the critical view of safety (CVS) principles.

Violence in intimate relationships (IPV) is understood as one of the most common forms of violence, being maintained by cultural habits and customs, and legitimized from generation to generation. This study aims at a comprehensive review of the literature on IPV, the relationship between the stages of change, maintenance factors, and the decision to keep or leave the violent partner. A comprehensive literature search was conducted to identify journal articles focused on IPV, following online databases as well as a manual review from relevant peer-reviewed journals.
