The esophago-pericardial fistula is a rare disease that causes a rather high mortality. Up to now less than 70 cases were published in the literature. The authors report an esophago-pericardial fistula caused by recurrent esophageal tumor one year after resection in the lower third esophagus for esophageal cancer. With conservative treatment the patient was kept alive for a month. No case report can be found in the literature of an esophago-pericardial fistula of the same etiology.

Purulent bacterial pericarditis is a rare and progressive infection with a high mortality. It is rarely due to , a commensal bacteria found in the oral cavity, gastrointestinal tract, and the genitourinary tract. Here we present a 71-year-old man with history of esophageal adenocarcinoma, status post distal esophagectomy and proximal gastrectomy 2 years prior, who developed cardiac tamponade secondary to primary purulent bacterial pericarditis in the absence of an esophago-pericardial fistula.

Purulent pericarditis secondary to esophago-pericardial fistula is a serious complication that has been previously reported in patients with esophageal cancer treated with radio/chemotherapy and esophageal stenting. However, the presence of esophago-pericardial fistula as the first manifestation of advanced carcinoma of the esophagus is exceedingly infrequent. We report the case of a 61-year-old male who presented with sepsis, cardiac tamponade and septic shock who was found to have an esophago-pericardial fistula secondary to squamous carcinoma of the esophagus.
