We report the case of an adult male presenting in shock caused by spontaneous rupture of a pathologically normal spleen. One day before his presentation he had received a single subcutaneous injection of enoxaparin. This was the patient's only clearly identifiable risk factor for spontaneous splenic rupture. Nontraumatic splenic rupture should be included in the differential diagnosis of shock.

After describing the anatomy of the spleen and the most important immunohistochemical stains for identifying cellular constituents of the normal splenic compartments, etiologies of splenomegaly and the diagnostic approach for spleen biopsies are discussed using the example of a North African patient with a recent migration background and sudden fever. The focus is on infectious diseases and the morphology and molecular features of hematolymphoid neoplasms, particularly the primary "splenic B‑cell lymphomas" according to the World Health Organization (WHO) classification. The importance of clinicopathological correlations and interdisciplinary cooperation in splenic pathology is emphasized.

Hereditary hemochromatosis occurs due to genetic mutations, namely, cysteine-to-tyrosine substitution at amino acid 282 (C282Y) and histidine-to-aspartic acid substitution at 63 (H63D) mutations. The role of H63D mutation in hemochromatosis is less clear, and its penetrance is low even in homozygotes. Therefore, iron overload in H63D heterozygotes is extremely rare and scarcely reported.
