Objectives: to evaluate preoperative clinical, surgical and instrumental variables as predictors of postoperative cardiac events in patients undergoing different types of elective major vascular surgery.
Material And Methods: on the basis of an algorithm including clinical and test echocardiographic data, we prospectively stratified 604 consecutive patients into low, intermediate and high-risk groups. The value of the variables in predicting postoperative cardiac events was assessed by means of multivariate analysis.
Results: there were 16 major postoperative cardiac events and six of 16 postoperative deaths were cardiac related (1%). Significant predictors of cardiac complications were unrecognised myocardial infarction (odds ratio - (OR) 5.6), coronary artery disease (OR 2.5), severe hypertension (OR 2.1) and peripheral vascular surgery (OR 1.9). In the intermediate-risk group, the best correlates with cardiac complications were unrecognised myocardial infarction (OR 3.3) and diabetes (OR 2.5).
Conclusions: our results suggest the importance of identifying patients with unrecognised ischaemic heart disease and of using aggressive perioperative protocols for managing diabetic patients undergoing peripheral vascular procedures.

Background: Repair of the regurgitant bicuspid aortic valve is an attractive alternative to valve replacement. Although good long-term outcomes have been reported, postoperative aortic stenosis remains a major late cause of repair failure in bicuspid aortic valves. Sinus plication is effective for creating a more symmetrical commissural angle, leading to a decrease in the mean transvalvular pressure gradient.

A young man presented with complaints of angina on exertion and dyspnoea on exertion for the last 3 months. On evaluation, he was found to have a cystic mass in the left ventricle with severe left ventricular systolic dysfunction. A cardiac MRI revealed a multiloculated mass in the left ventricle with multiple septa with internal enhancement and CT coronary angiography revealed compression of a coronary artery by the cystic mass.

Background: Transcatheter closure of percutaneous paravalvular leak (PVL) is a technically challenging procedure, especially after surgical mechanical valve replacements (SMVR), as the risk of interference with the prosthetic valve discs and the complex interventional techniques required for mitral PVL closure. Our study was designed to review the results with transcatheter closure of PVL after SMVR.
Methods: From January 2018 through December 2023, a total of 64 patients with PVL after SMVR underwent transcatheter closure with the help of preoperative 3-dimensional printing model and simulator for image evaluation.

This retrospective study evaluates the clinical impact of perioperative multimodal analgesia in the minimally invasive treatment of severe blunt chest trauma with hemopneumothorax using a thoracoscopic Ni-Ti shape memory embracing plate. A total of 100 patients with severe blunt chest trauma and moderate to severe hemopneumothorax treated at Hanyang Hospital affiliated with Wuhan University of Science and Technology from January 2019 to January 2022 were enrolled. Patients were divided into 2 groups: a control group (50 patients) receiving patient-controlled intravenous analgesia (PCIA), and a study group (50 patients) administered a multimodal analgesia regimen.
