Service de Pneumologie, CHU, bd Fleming, 25000 Besançon.

Published: November 2000

A 47-year-old woman, without significant past medical history, presented an acute dyspnea with hypoxia, marked pulmonary arterial hypertension (PAH) and signs of right heart failure. Chest x-ray showed a moderate dilatation of the right heart cavities. Pulmonary embolism was suggested. After detailed questioning and complete explorations, a bird hypersensitivity pneumonitis (HP) was demonstrated. This case illustrates a misleading presentation of an acute form of HP consisting of apparently isolated PAH.

As the population of adults with congenital heart disease (ACHD) grows, there also grows an expanded need for non-invasive surveillance methods to guide management and intervention. A multimodal imaging approach layers complementary insights from echocardiography, computed tomography (CT), magnetic resonance imaging (MRI), and other modalities into a clinician's view of patient physiology. Merely applying strategies from acquired adult cardiac disease would be inadequate and potentially misleading.

The diagnostic process and discrimination of mucosal lesions present a formidable challenge for numerous clinicians, primarily attributable to the common overlap of clinical manifestations observed across various categories, including infectious, autoimmune, connective tissue, and systemic vascular inflammatory diseases. In cases of mucosal lesions, syphilis presents distinctive characteristics that can help clinicians differentiate it from other conditions. The most common manifestation of primary syphilis is mostly a painless, firm, indurated ulcer known as a chancre, which typically appears at the site of inoculation, with enlargement of regional lymph nodes.
