Two cases of colorectal cancer complicating Crohn's disease and review of the literature.

Langenbecks Arch Surg

Chirurgische Abteilung im Elisabeth Krankenhaus Köln-Hohenlind, Cologne, Germany.

Published: November 2000

Two cases of colorectal carcinoma complicating Crohn's disease are reported and the relevant literature is reviewed. It is important to be aware of this increasing complication and the specific group of patients at risk. Special attention to possible malignant transformation could lead to earlier diagnosis and treatment.

Introduction: Inflammatory bowel disease (IBD) patients have an increased risk of developing colorectal cancer. High-risk colorectal colitis-associated neoplasia (HR-CAN) can be difficult to treat using traditional endoscopic resection methods. Aim of the study is to evaluate the outcomes of endoscopic submucosal dissection (ESD) on IBD patients with HR-CANs.

Dynamic spectrum-driven hierarchical learning network for polyp segmentation.

Med Image Anal

January 2025

Department of Computer and Data Science and Department of Biomedical Engineering, Case Western Reserve University, Cleveland, USA.

Accurate automatic polyp segmentation in colonoscopy is crucial for the prompt prevention of colorectal cancer. However, the heterogeneous nature of polyps and differences in lighting and visibility conditions present significant challenges in achieving reliable and consistent segmentation across different cases. Therefore, this study proposes a novel dynamic spectrum-driven hierarchical learning model (DSHNet), the first to specifically leverage image frequency domain information to explore region-level salience differences among and within polyps for precise segmentation.

A prospective study of methylated ctDNA in patients undergoing treatment for liver metastases from colorectal cancer.

Background: Decision regarding local treatment of colorectal liver metastases (CRLM) is a multidisciplinary assessment, and liver intervention should be performed when the metastases are deemed resectable. There is no standard biomarker to aid neither this decision nor the postoperative treatment decisions. The present prospective, observational study aimed to investigate the potential clinical utility of a combined tumor-specific and organ-specific methylated circulating DNA assay in the perioperative setting of CRLM.

Background: One in five sebaceous tumour (ST) patients may have Lynch syndrome (LS), a hereditary cancer predisposition. LS patients benefit from cancer surveillance and prevention programmes and immunotherapy. Whilst universal tumour mismatch repair (MMR) deficiency testing is recommended in colorectal and endometrial cancers to screen for LS, there is no consensus screening strategy for ST, leading to low testing rates and inequity of care.

Department of Colorectal Surgery, National Cancer Center Hospital East, 6-5-1 Kashiwanoha, Kashiwa, Chiba, 277-8577, Japan.

Lateral lymph node dissection (LLND) is getting global attention as an a surgical option to reduce local recurrence in locally advanced rectal cancer. As the transanal total mesorectal excision (TaTME) is gaining popularity worldwide, a novel LLND approach was established adopting a two-team approach that combines the transabdominal and transanal approaches using the TaTME technique. This narrative review describes the advantages, anatomical landmarks, surgical techniques, and pitfalls of transanal LLND (TaLLND).
