Fibrosing mediastinitis is a rare chronic inflammatory condition characterized by excessive fibrotic process in the mediastinum. Although various infectious processes are more common causes, it can also be related to several malignancies. We report a case of a 28-year-old woman with fibrosing mediastinitis related to an aggressive primary gray-zone lymphoma causing complete occlusion of the superior vena cava (SVC) and the innominate veins.

At the age of 59 years, she was diagnosed with neurosarcoidosis at another hospital, and she received oral glucocorticoid therapy for 1 year. Her serum angiotensin-converting enzyme (ACE) and 1, 25-dihydroxyvitamin D levels were elevated.

Abdominal pain is a common presenting symptom among patients visiting the hospital. A wide range of differential diagnoses are associated with this presentation, some of which are more uncommon than others, and require a higher degree of clinical suspicion and radiological excellence to diagnose. Although clinicians rely on physical assessment, examining a patient who is agitated and non-cooperative sometimes limits the physical exam findings, making these diagnoses even more challenging.

A 60-year-old female with complaints of cough and breathing difficulty also presented with pain and swelling in her left leg. Radiological investigations suggested a double primary in the lung and leg; histopathology and immunohistochemistry (IHC) confirmed the lesion in the leg to be metastatic from the lung primary.
