Standardization of veloergometry for functional assessment of oxygen transport in patients with ischemic heart disease (IHD) was studied using cardiopulmonary complex Siregnost FD 88S (Siemens, Germany) and spiroveloergometry. Standard veloergometry protocol was applied for 30 healthy and 55 IHD males. The methodological approach and validity of application of standard veloergometry protocol in IHD patients are discussed. The regression equations for indirect estimation of maximal oxygen consumption by the time of action of the standard veloergometry protocol in healthy subjects and IHD males have been devised on the basis of the data obtained at linear regression analysis. The method provides an approximate indirect estimate of maximal oxygen consumption in the absence of gasoanalytical equipment.

The aim of our study was to assess the anti-ischemic and cardiouprotective efficiency of the activator of potassium channels nicorandil in patients with stable angina during long-term intake on the background of standard therapy of coronary heart disease (CHD), including aspirin, rosuvastatin, valsartan and bisoprolol in comparison with standard therapy and long-acting form of nitrates. We compared the dynamics of nitroglycerine consumption, frequency of angina attacks, carried veloergometry, daily monitoring of an electrocardiogram by Holter, echocardiography, assess the state of lipid and carbohydrate metabolism, were determined concentrations of certain markers of systemic inflammation. The results showed that the inclusion of nicorandil into the standard therapy CHD leads to more significant changes in indicators of clinical evaluation of angina pectoris, myocardial remodeling, inflammatory markers.

Prevalence of minor and major complications, side effects of stress echocardiography (stress-echoCG) was studied in a group of 1359 patients with suspected ischemic heart disease (IHD). Dipyridamole/atropine test was carried out in 184, dobutamine/atropine test - in 231, transesophageal atrial pacing (TEAP) - in 154, veloergometry (VEM) in half sitting position - in 122, and combination stress-echoCG - in 668 patients (dipyridamole/TEAP, n=151; dipyridamole/dobutamine, n=162; dipyridamole/paired TAP, n=112). We classified ventricular and supraventricular extrasystoles, head ache, nausea, muscular tremor as side effects, short paroxysms (less than 2 min) of hemodynamically insignificant supraventricular and ventricular tachycardia - as minor complications, and development of acute coronary syndrome, ventricular fibrillation - as major complications.
