Management is an increasingly important issue for many doctors. If doctors wish to influence resource allocation, they must involve themselves in health service management. This article describes the results of an enquiry action learning project involving six doctors. As part of the project, clinical directors and their business managers were interviewed. In addition, the Police Force and BAA (formerly the British Airport Authority) were visited and their management structure, out-of-hours activities and planning for emergencies assessed. This article examines the reasons why many doctors do not involve themselves in management, such as increased time commitment and negative peer pressure, and suggests some solutions to these problems, including the need for a wider understanding of the role of clinical directors. It also considers how some organizations are already starting to address these issues, and how both doctors and hospitals can benefit from greater involvement of doctors in health service management.

Background: Hispanic/Latinx older adults have increased risk of developing Alzheimer's disease, poor access to timely and quality dementia care, as well as limited access to caregiver support and interventions. We addressed these structural barriers at a local level in central Virginia in order to improve disparities in risk, early detection, and care.

Method: Systematic expansion of services was undertaken by establishing a Spanish neuropsychological clinic, providing personalized scheduling services by providers to ensure appropriate follow-up after referral is received, engaging in dementia specific community talks through a broader health system initiative (UVA Latinx Health Initiative), and facilitating dementia care coordination services for caregivers.

Background: Resource-constrained rural areas face significant challenges in providing access to healthcare resources, especially for older adults, including those living with Alzheimer's disease and related dementia (ADRD). We seek to address these gaps by equipping six rural community sites in New Hampshire and Maine with tele-rehabilitative equipment. Background: We have co-produced with carers of people with dementia (hereafter carers) a culturally tailored iSupport Virtual Assistant (VA), namely e-DiVA, to support English-, Bahasa- and Vietnamese-speaking carers in Australia, Indonesia, New Zealand and Vietnam. The presented research reports qualitative findings from the e-DiVA user-testing study.

Method: Family carers and healthcare professionals working in the field of dementia care were given the e-DiVA to use on their smartphone or handheld device for 1-2 weeks.
