Introduction: Echinococcus granulosus, a tapeworm, is responsible for causing hydatid disease. Hydatid cysts rarely affect the musculoskeletal system, occurring in less than 5% of cases. This case report presents the clinical presentation, diagnosis, and management of muscle echinococcosis.

Background: Cystic echinococcosis mainly affects the liver and lungs, in which the larvae from the microvascular wall in the liver pass to the lungs and then to the blood circulation and settle in any tissue or organ.
Objectives: The objective of this study was to report the unusual location of hydatid cysts in infected patients in Sulaimaniyah City, Iraq.
Patients And Methods: This retrospective case series study enrolled 13 patients.

Intramuscular hydatid cysts are an uncommon presentation of echinococcosis, with most cases occurring in the liver and lungs. This case report describes an 81-year-old male who presented with a painless, progressively enlarging swelling in the right thigh noticed over the past year. The patient's history included trauma to the area from an animal-related incident five years earlier.

Background: Hydatid disease, also known as echinococcosis, is a zoonotic parasitic infection caused by the larvae of the Echinococcus tapeworm. It is endemic in various regions worldwide, particularly in rural areas of countries in southern South America, Central Asia, China, parts of Africa, the Mediterranean, and parts of the Middle East. The disease primarily affects the liver (60-70% of cases) and the lungs (10-25% of cases), but it can involve any organ, including the brain, bones, and rarely the pelvic region, as seen in our case report.

Echinococcosis, or hydatid disease, is a parasitic infection caused by a cestode from the Taeniidae family, mainly by Echinococcus multilocularis or granulosus. It is predominantly seen in the lungs or the liver. The hydatid disease rarely manifests as a palpable mass in the muscles.
