[Acardiac acephalic fetus and twin pregnancy. Report of a case].

The present case describes an acardiac, acephalic fetus, diagnosed by ultra-sound, during the 33rd gestational week of an insulin dependent diabetic parturient, who had developed toxemia gravida. The diagnosis had been confirmed by the autopsy. The differential diagnosis from an in-utero death of the second twin had been raised. The acardiac acephalic foetus is a rare congenital malformation observed during the monozygotic twin pregnancy. The second fetus being normal, our approach consisted in the clinical observation of the parturient, followed by cesarean section delivery the 4th day of the 36th gestational week to avoid worsening of toxemia gravida.

Rationale: The preferred method for multifetal pregnancy reduction (MFPR) is a transabdominal intrathoracic or intracranial injection of potassium chloride (KCl). However, in monochorionic multiple pregnancies (MMPs), especially in monoamnionic multifetal pregnancies, selective feticide by this method is often associated with miscarriage of the remaining fetuses. Selective fetal reduction in MMPs by blood flow ablation using radiofrequency ablation or fetoscopic laser surgery may improve survival of the remaining fetus.

Single umbilical artery is the most common congenital abnormality of the umbilical cord and is seen in 4-11% of twin pregnancies. It is usually associated with intrauterine growth retardation, preterm labour, small-for-dates and other structural anomalies. We report here a case of twin reversed arterial perfusion sequence with transposition of great arteries in the pump twin. Color Doppler and fetal echocardiographic examination revealed an acardiac, anencephalic twin without upper limbs and a pump twin with transposition of great arteries. To our knowledge despite multiple anomalies reported in the acardiac twin, major anomalies, especially of a cardiac type are extremely rare and not reported in the pump twin, which maintains circulation.

This report details one case occurring in a twin pregnancy in a 19 year old primigravid patient who has spontaneously aborted at 21 weeks gestation. Both twins were males, one of the them weighted 280 g and was acephalic.
