The neuropsychological assessment of U.S. Air Force pilots presents several unique problems given their relatively high cognitive functioning. The U.S. Air Force currently has a baselining procedure whereby student pilots undergo computerized cognitive assessment. The intent of this assessment is to archive premorbid data against which to compare potential future postinsult performance. The current work provides the background, clinical methods, and data needed to assess pilots who have suffered cortical insult such as trauma, disease, or exposure to toxins. Methods are delineated for pilots with premorbid testing and for pilots without such testing.

A young male with no known addictions and comorbidities presenting with recurrent clonic-myoclonic movements, initially localized to the left corner of the mouth and left upper limb, evolving into epilepsia partialis continua, despite appropriate sequential antiepileptic medications, subsequently progressed to refractory status epilepticus. He was tested positive for HIV infection and his neuroimaging revealed nonenhancing lesions, a novel finding in HIV-related encephalitis. We managed him with intravenous immunoglobulin along with multiple antiepileptic medications and highly active antiretroviral therapy (ART), and he exhibited a rapid clinical recovery over 3 weeks.

Asymmetric photoreactions catalyzed by chiral ketones.

RSC Adv

Shaanxi Province Key Laboratory of Catalytic, School of Chemical and Environmental Science, Shaanxi University of Technology Hanzhong 723001 P. R. China

Asymmetric catalytic reactions are essential for synthesizing chiral drugs and fine chemicals, with their stereoselectivity influenced significantly by interactions between catalysts and substrates. Ketone catalysts have garnered considerable attention in the realm of asymmetric photoreactions because of their highly controllable structures, ease of availability, and environmental friendliness. This review highlights the application of various reported ketone catalysts in a range of asymmetric photoreactions, including [2 + 2] photocycloaddition, photoderacemization, photochemical rearrangement, asymmetric electrophilic amination, and asymmetric alkylation of aldehydes.

In case of venous route abnormality during a leadless pacemaker (LP) implantation, it can be challenging if we still performed via the predesigned femoral vein. We report a patient with normal preoperative laboratory and image results, but azygos continuation of the inferior vena cava (IVC) was suspected during the procedure. Then, we decided to change the implantation strategy, the LP implantation was successfully performed via right jugular vein instead of the classical IVC route.
