Spontaneous ureteral rupture is a rare cause of acute abdominal pain, particularly unusual during pregnancy or the post-partum period. While pregnancy-related changes like ureteral compression and dilation may play a role, no definitive mechanisms have been established. Clinicians should suspect ureteric injury in post-partum patients with free pelvic fluid.

Background: Open pelvic fractures are rare but represent a serious clinical problem with high mortality rates. Acute mortality is often associated with hemorrhage, whereas delayed mortality is most often associated with sepsis and multiple organ failure. Adrenal haemorrhage in pregnancy is rare but can lead to significant maternal and foetal morbidity if unrecognised. We present the case of a 25-year-old woman in her second pregnancy, who was admitted at 34+4 weeks of gestation with severe abdominal pain. Despite initial unremarkable assessments, further imaging revealed a left adrenal haemorrhage, with evidence of prior right adrenal infarction, resulting in primary adrenal insufficiency.
