Objective: Lengthening of the patellar tendon to normalize patellar height and improve knee flexion deficits.
Indications: Flexion deficits in combination with patella baja (Caton index < 0.6).

Objective: Inferior pole patellar fractures (IPPFs) pose a significant challenge due to their complex fracture patterns and high risk of complications associated with current treatment methods. This study aims to (1) characterize the fracture patterns of IPPFs using fracture mapping and (2) compare the biomechanical stability and clinical outcomes of treatment with anchor suture with patellar cerclage versus Kirschner-wire tension band combined with patellar cerclage.
Methods: (1) A retrospective analysis was conducted on 61 patients with IPPF.

Aim: Our study aimed to evaluate the effectiveness of ultrasound-indicated cerclage in singleton pregnancies with cervical shortening (<25 mm), excluding those with a history of preterm birth (PTB) and infection/inflammation.
Materials And Methods: Among the 1556 women admitted for a cervix measuring <25 mm via transvaginal ultrasound at Juntendo University Urayasu Hospital between January 2001 and December 2023, our study focused on 47 singleton patients with no prior history of PTB. After receiving information on both risks and benefits, 25 patients opted for ultrasound-indicated cerclage (cerclage group), while 22 chose expectant management (expectant management group).

Objective: A single-center randomized trial showed improved latency with use of indomethacin and cefazolin (I/C) during and following exam-indicated cerclage (EIC). The same center recently published a pre/post comparison demonstrating similar results. This research aimed to validate the protocol in a different setting.

Cervical cerclage is a widely used intervention to prevent preterm birth in high-risk pregnancies. However, cerclage is associate with risks, including preterm premature rupture of membranes and subsequent complications, such as chorioamnionitis. Our review evaluates the evidence for immediate removal (ie, removal at the time of diagnosis) vs retention of cervical cerclage (ie, removal when clinically indicated) after preterm premature rupture of membranes, focusing on optimizing neonatal outcomes and minimizing maternal and fetal complications.
