Anterior neopharyngeal pseudodiverticula are out-pouchings of the neopharyngeal lumen in laryngectomized patients which can cause postoperative dysphagia. In this study 20 laryngectomized patients were examined endoscopically and with barium swallow to determine the frequency of pseudodiverticulum formation, the correlation with clinical symptoms, and to evaluate the best modality for diagnosis. In 12 patients an anterior neopharyngeal pseudodiverticulum was present. Of these patients eight complained of dysphagia. The barium swallow showed more clearly the pseudodiverticula than laryngoscopy and gave additional information on functional implications. All symptomatic patients were successfully operated on with endoscopic laser therapy.

Objectives: To review the literature on neo-vallecula diagnosis and management and to report our findings regarding 3 patients who developed neo-vallecula in the context of free-flap pharyngeal reconstruction following total laryngectomy.
Methods: This case series reports three patients who developed a neo-vallecula following a laryngectomy and free-flap pharyngeal reconstruction. All three patients were treated with a CO laser endoscopic procedure.

Postlaryngectomy dysphagia is a common occurrence and can be a source of emotional distress that results in a decrease in quality of life among a patient population that is already exposed to considerable morbidity. One etiologic source that is less commonly reported as a source for postlaryngectomy dysphagia, and perhaps overlooked, is an anterior neopharyngeal diverticulum. Herein, we describe a postlaryngectomy dysphagia caused by a neopharyngeal diverticulum masking as velopharyngeal insufficiency of liquids.
