A need for long-term ventilation in the home has created a demand for home care services that has been a source of growth for an industry. Evaluation of the patient and family who will guarantee successful home care requires careful psychological and psychiatric evaluation. Beyond this evaluation, long-term success is possible if appropriate financial resources are available. The implementation of successful home care is best done by a team consisting of primary physician, primary ICU nurse, social worker, psychiatrist, and home care respiratory therapist. An appropriate classification of patients based on goals of care, as well as the study of the incidence and prevalence of associated disorders, will be helpful in the future allocation of resources.

Prior findings suggest a greater therapeutic focus on strengths is associated with a higher likelihood of successful treatment discharge in youth public mental health treatment. To build upon previous research, the present study examined whether and to what extent therapeutic focus on strengths was associated with changes in day-to-day functioning and treatment progress over the course of therapy. This study used multilevel modeling techniques to analyze 12 consecutive years of standardized routine clinical service data from a public mental health care system which served youth and families who were typically from underserved and low-income backgrounds.

Background And Objective: While standard doses of adjuvant fluoropyrimidine-based chemotherapies are generally safe for most patients, the risk of severe adverse drug reactions (ADRs) is increased for those with dihydropyrimidine dehydrogenase deficiency (DPYD), a genetic variation that affects drug metabolism. The objective of this study was to examine the cost effectiveness of offering DPYD pharmacogenetic-guided care, where genetic testing informs personalized dosing versus the current standard of care (SoC), which involves administering fluoropyrimidine-based therapies without prior genetic screening, for local or metastatic breast cancer patients in Qatar.

Methods: We developed a two-stage decision analysis, with an analytic tree model over a 6-month period, followed by a life-table Markov model over a lifetime horizon.
