It is possible to predict the likelihood of postoperative blood transfusion using patient variables. The information could be used to target the high-risk group for intervention(s) that would subsequently reduce the risk of exposure to allogeneic blood. The cost of several of the alternatives to allogeneic blood precludes their use on all surgical patients, and hence the need to identify those patients who are most likely to benefit from such therapy. Further research is needed to assess the impact of predictive models on the use of allogeneic blood and costs. Research is also needed to clarify the impact of blood transfusion on perioperative morbidity and mortality, in patients with and without heart disease. If such an association does indeed exist, then an optimal hemoglobin or transfusion threshold is needed to guide clinicians in caring for their patients.

Despite the vast heterogeneity of myelodysplastic neoplasm (MDS), treatment options are limited and an allogeneic hematopoietic stem cell transplantation (allo-HSCT) remains the only curative approach. While, subsequently, allo-HSCT is the treatment of choice in fit high-risk MDS patients younger than 70 years, it should only be considered in young and fit low-risk MDS patients who suffer from severe and life threatening cytopenias, and fail all available conservative treatment options. With the increasing use of mismatched or haploidentical donors, the majority of MDS patients will have a suitable donor available, however, matched donors should still be preferred if rapidly available.

In the randomized, double-blind, phase 3 GRAPHITE study (NCT03657160), anti-αβ integrin antibody vedolizumab showed greater efficacy than placebo for prevention of lower-gastrointestinal (GI) acute graft-versus-host disease (aGVHD) after unrelated allogenic hematopoietic stem cell transplantation (allo-HSCT). This post hoc analysis assessed the efficacy and safety of vedolizumab versus placebo for lower-GI aGVHD prevention in Japanese and non-Japanese patients, when added to standard GVHD prophylaxis (calcineurin inhibitor + methotrexate/mycophenolate mofetil + / - anti-thymocyte globulin [ATG]). The analysis included 35 (18 vedolizumab-treated, 17 placebo-treated) Japanese and 298 (150 vedolizumab-treated, 148 placebo-treated) non-Japanese patients.

Cytomegalovirus (CMV) infection post-hematopoietic cell transplantation (HCT) remains a significant cause of morbidity and mortality. While letermovir prophylaxis is available for CMV-seropositive recipients, optimal donor selection for CMV-seronegative recipients remains unclear, with donor age often prioritized over CMV serostatus. We investigated the relative impact of donor age and CMV serostatus in CMV-seronegative recipients (n = 1013) with either CMV-seropositive (n = 318) or CMV-seronegative donors (n = 695), who underwent HCT with HLA-matched sibling donors with calcineurin inhibitor-based or post-transplant cyclophosphamide (PTCy)-based graft-versus-host disease prophylaxis, or haploidentical donors with PTCy.
