A 58-year-old woman underwent aortic valve replacement. On the second postoperative day the patient referred a sharply chest pain, and an emergent coronary angiography revealed total occlusion of the right coronary artery. An intra-aortic ballon pump was placed and the patient underwent emergent off-pump coronary revascularization of the right coronary artery.

Article Synopsis
  • Cardiovascular issues from tertiary syphilis are rare but can lead to serious health problems and even death.
  • A 36-year-old man was admitted with a severe heart attack and was found to have significant heart dysfunction along with evidence of syphilitic infection.
  • Treatment involved urgent stenting of a blocked coronary artery and antibiotics, highlighting that syphilitic aortitis can present suddenly during acute medical emergencies.
  • Long-term monitoring is essential to prevent complications like restenosis due to ongoing infection.

In a multiple-vessel lesion of the coronary bed, pronounced accompanying pathology, low ejection fraction of the left ventricle, obesity, previously performed operation of coronary aortic bypass grafting may be causes of refusal from revascularization by means of coronary bypass grafting. A transcutaneous coronary intervention (TCI) in this patient cohort is also associated with the risk of an unfavourable outcome in case of a technically complicated procedure and a large volume of the myocardium supplied with blood by the target arteries. Haemodynamic support of such high-risk TCI by means of intra-aortic balloon contrapulsation or a left-ventricle assist device is associated with a series of shortcomings which are removed by means of a biventricular bypass used in a combination with extracorporeal membranous oxygenation.

Aims: to determine immediate and long-term results, safety and efficacy of one-stage PCI of LMCA and infarct-related LAD or CxA in STEMI.

Methods: 81 patients with STEMI treated with one-stage PCI of LMCA and infarct-related LAD or CxA were included into the study. LAD was infarct-related in 53 (65.

Extracorporeal membrane oxygenation (ECMO) is becoming a gold standard in acute heart failure, not responsive to inotrops and intra-aortic balloon contrapulsation. This diffusion is due to the possibility to implant it through peripheral cannulation and to perform long-time assistance. Nevertheless, this technique implies some problems concerning inferior limb perfusion.
