Thoracic manifestations of internal pancreatic fistulas caused by chronic pancreatitis are rare conditions. The three main types of these manifestations are mediastinal pseudocysts, pancreatico pleural fistulas and pancreaticobronchial fistulas. We report on one patient with the clinical presentation of all three thoracic internal pancreatic fistulas with a communication to a pseudoaneurysm of the splenic artery caused by chronic alcohol-related pancreatitis. Conservative therapy over four weeks was not successful. Resection of the pseudoaneurysm, debridement of the mediastinal pseudocyst and duodenum preserving resection of the pancreas treated all complications and prevents recurrence in this patient with chronic pancreatitis.

Spontaneous non-aneurysmal aortic rupture is rare and is usually attributed to penetrating aortic ulcers, infections, tumor infiltrations, or inflammatory and collagen diseases. Chronic rupture is infrequent but extremely rare in non-aneurysmal aortas, which makes diagnosis difficult because the absence of an aneurysm can mislead the physician to rule out rupture. Here, we describe the case of an 85-year-old male, who was undergoing oncological investigation for weight loss, inappetence, and back pain.

An 86-year-old female was taken to hospital with complaints of general malaise and anorexia. Echocardiography showed an abnormal space between the ventricles, extending to the back of the left atrium, with a shunt from the left ventricle into both that abnormal space and the right ventricle. The next morning, the patient had a large amount of tarry stool and progressive anemia.

A 66-year-old male patient underwent repair using the sutureless technique for an oozing-type cardiac rupture associated with myocardial infarction of the anterior left ventricular wall. On postoperative day 10, echocardiography revealed a thrombus in the apex region, and anticoagulation therapy was started with warfarin. The thrombus disappeared two months postoperatively.
