Successful Percutaneous Closure of Gerbode Defect and Right Atrial-Aortic Fistula Following Infective Endocarditis.

We report a case of infective endocarditis with a septal abscess that was complicated with abnormal blood flow from the left ventricle to the right atrium (Gerbode defect) along with abnormal blood flow from the aorta to the right atrium (atrial-aortic fistula). This is the first reported case of successful correction of both defects by a percutaneous approach.

Sum frequency generation spectroscopy of fluorinated organic material-based interfaces: a tutorial review.

Molecular interactions at interfaces have a significant effect on the wetting properties of surfaces on a macroscale. Sum frequency generation (SFG) spectroscopy, one of a few techniques capable of probing such interactions, generates a surface vibrational spectrum sensitive to molecular structures and has been used to determine the orientation of molecules at interfaces. The purpose of this review is to assess SFG spectroscopy's ability to determine the molecular orientations of interfaces composed of fluorinated organic molecules.

Tail feather strength in tail-assisted climbing birds is achieved through geometric, not material change.

Birds encompass vast ecomorphological diversity and practise numerous distinct locomotor modes. One oft-cited feature seen in climbing birds is an increase in tail 'stiffness', yet it remains unclear to what extent these feathers are altered, and the specific mechanism by which differences in functional performance are attained. We collected a broad taxonomic sample of tail feathers (6525 total, from 774 species representing 21 avian orders and ranging in size from approximately 3 g to greater than 11 kg) and present data on their material properties, cross-sectional geometry and morphometrics.

Introduction: Firearm-related deaths have become the leading cause of death in adolescents and children. Since the Sutherland Springs, TX mass casualty incident (MCI), the Southwest Texas Regional Advisory Council for trauma instituted a prehospital whole blood (WB) program and blood deployment program for MCIs.

Methods: The program was adopted statewide by the Texas Emergency Medical Task Force, of which Southwest Texas Regional Advisory Council is the lead for Emergency Medical Task Force 8.

Clients with HIV infection have been described as both vulnerable and resilient. In addition, the conceptualization of clients with HIV infection has evolved. The purpose of this study is to examine this relationship among a sample of people with HIV infection.

Percutaneous coronary intervention and the management of acute coronary syndromes in patients with von Willebrand disease.

Background: Von Willebrand disease (vWD) results from quantitative or qualitative deficiency of von Willebrand factor (vWF). The occurrence of myocardial infarction is very rare in patients with vWD. A few case reports of acute coronary syndrome (ACS) in vWD patients are present in the literature, but no definite management recommendations are available for such patients.

Background: Clients with HIV infection have been conceptualized as a resilient population. Although a few researchers have documented resilience among clients with HIV infection, a theory of resilience in the context of HIV infection has not been developed. The purpose of this study was to describe the process by which resilience occurs for clients in the context of HIV infection.

Background: Men who have sex with men (MSM) experience a number of health disparities including high rates of HIV from high risk sex. High rates of HIV infection are related to high risk sexual behaviors and multiple sexual partners. High rates of HIV infection in this population also may be related to high risk sexual behaviors that occur within the context of primary relationships.

Acquired pincer nail deformity associated with renal failure.

Pincer nail deformity represents a loss in the normal convex shape of the nail and has been reported in association with various diseases and medications. The authors report a man with end-stage renal disease due to bladder carcinoma who developed pincer nail deformity while on hemodialysis. Previous case reports cite diabetes as a suspected cause of pincer nail development in patients with renal failure.

SOMOS: evaluation of an HIV prevention intervention for Latino gay men.

Latino gay men face multiple barriers to human immunodeficiency virus (HIV) prevention, in particular a lack of intervention programs that integrate prevention messages with cultural norms and address issues of social marginalization from multiple communities (gay community and Latino community), homophobia and racism. In order to address these specific issues, a multilayered HIV intervention was designed to incorporate and integrate psychosocial and community factors through multiple session groups, social marketing and community presentations. Participants learned strategies for effective community leadership and were encouraged to provide HIV education and address internalized homophobia in their communities.

20-Hydroxycholecalciferol, product of vitamin D3 hydroxylation by P450scc, decreases NF-kappaB activity by increasing IkappaB alpha levels in human keratinocytes.

The side chain of vitamin D3 is hydroxylated in a sequential manner by cytochrome P450scc (CYP11A1) to form 20-hydroxycholecalciferol, which can induce growth arrest and differentiation of both primary and immortalized epidermal keratinocytes. Since nuclear factor-kappaB (NF-kappaB) plays a pivotal role in the regulation of cell proliferation, differentiation and apoptosis, we examined the capability of 20-hydroxycholecalciferol to modulate the activity of NF-kappaB, using 1,25-dihydroxycholecalciferol (calcitriol) as a positive control. 20-hydroxycholecalciferol inhibits the activation of NFkappaB DNA binding activity as well as NF-kappaB-driven reporter gene activity in keratinocytes.
