Article Synopsis
  • * A study analyzed immune cells from 41 male Fabry patients and 20 controls, revealing that FD patients show higher expression of specific markers (CD27 and CD28) in certain T cells, which correlates with the levels of harmful lipids (lysoGb3 and Gb3) in the blood.
  • * Treatment with agalsidase beta was found to increase Natural Killer cell proportions, suggesting potential new biomarker correlations that could enhance understanding and management of Fabry disease.

Respect of patient's autonomy is essential. So that patients are able to write their advance directives in case of a situation where they are unable to make decisions for themselves. Currently, few people have written advance directives.

Backgroud: Fabry disease (OMIM #301 500), the most prevalent lysosomal storage disease, is caused by enzymatic defects in alpha-galactosidase A (GLA gene; Xq22.1). Fabry disease has historically been characterized by progressive renal failure, early stroke and hypertrophic cardiomyopathy, with a diminished life expectancy.

Article Synopsis
  • Fabry disease is an X-linked disorder linked to a deficiency in an enzyme called alpha-galactosidase A, leading to classic and non-classic clinical phenotypes and was treated with enzyme replacement therapy (ERT) starting in 2001.
  • In a study of 103 patients (53 males) in the French cohort FFABRY, it was found that 40% of men and 8% of women developed antibodies against ERT, with a significant prevalence in males with specific mutations and classic phenotype.
  • The study showed that antibody levels, particularly IgG4 and IgG2, were correlated with treatment inhibition and plasma lysoGb3 levels, revealing an association between immune response and clinical outcomes in Fabry disease

Background: The natural history of nonfistulising perianal Crohn's disease (PCD) remains unknown.

Aim: To assess the long-term outcome of anorectal strictures.

Methods: All outcomes of PCD patients with anorectal stricture followed in a single unit from 2005 to 2013 were reviewed, as well as subsequent therapeutic management.

Lenalidomide is an immunomodulating drug structurally similar to thalidomide. It is indicated for patients with relapsing or refractory multiple myeloma in combination with dexamethasone, and for patients with myelodysplastic syndromes associated with a deletion 5q cytogenetic abnormality. It is also used to treat other myelodysplastic syndromes such as myelofibrosis and lymphoma.

Endoscopic evaluation after chemoradiotherapy (CR) is impossible with an esophageal stent in place. The main study objective was to evaluate self-expanding plastic stent (SEPS) removal post-CR. Secondary end-points were the improvement of dysphagia and patients' quality of life.

Background: The risks associated with pancreaticoduodenectomy (PD) in elderly patients continue to be debated. The aim of our study was to assess the incidence of death and postoperative complications following PD and identify the risk factors in patients >75 y.

Study Design: All patients who underwent PD between January 2000 and September 2009 were analyzed retrospectively.

Background: The mean small-bowel capsule reading time is about 60 min, and shortening this reading time is a major aim.

Aim: To evaluate the efficiency of the "Quick-view" detection algorithm.

Methods: Multicentre prospective comparative study.

Background And Study Aims: Patients with Lynch syndrome are at increased risk of developing small-bowel adenocarcinoma, which usually has a bad prognosis and needs to be diagnosed early. Our aim was to evaluate the yield of capsule endoscopy and CT enteroclysis in this situation.

Patients And Methods: We performed a prospective, blinded, comparative study of capsule endoscopy and CT enteroclysis in five academic centers.

Background: Endoscopic interventions are usually very challenging in patients with anatomic changes caused by earlier GI surgery. The recent development of the double-balloon enteroscopy (DBE) system creates an opportunity to reach the biliary tract and the pancreatic duct in postsurgical conditions.

Objective: To report a further application of DBE in surgical patients; namely, extraction of pancreatic stents placed in pancreaticojejunal anastomosis during pancreaticoduodenectomy.

Background And Study Aim: A video capsule similar to that used in small-bowel capsule endoscopy is now available for esophageal exploration. The aim of our study was to compare the accuracy of upper endoscopy (esophageal gastroduodenoscopy [EGE]) with esophageal capsule endoscopy (ECE) in patients at risk of esophageal squamous cell cancer (SCC).

Patients And Methods: 68 patients at risk of SCC secondary to a history of head and neck neoplasia were included in this comparison of techniques for detecting SCC and dysplasia.

Objectives: Esophagogastroduodenoscopy (EGD) is the standard method for the diagnosis of esophago-gastric varices. The aim of this prospective multicenter study was to evaluate the PillCam esophageal capsule endoscopy (ECE) for this indication.

Methods: Patients presenting with cirrhotic or noncirrhotic portal hypertension underwent ECEfollowed by EGD at the time of diagnosis.

Aspiration is a rare complication of video-capsule endoscopy. We report a case of asymptomatic aspiration of the capsule in a 90-year-old man. The diagnosis was obtained by the images taken by the capsule.

Background: Capsule endoscopy (CE) is the first procedure to explore the small bowel in obscure GI bleeding (OGB).

Objective: To evaluate the role of bowel preparation with oral sodium phosphate (NaP) in this indication.

Design: A prospective multicenter, controlled, randomized, blind study.

Background And Study Aim: Polyp miss rates during colonoscopy have been calculated in a few tandem or back-to-back colonoscopy studies. Our objective was to assess the adenoma miss rate while limiting technique or operator expertise biases, i. e.

Background: Several randomized studies have shown that colorectal cancer (CRC) screening by faecal occult blood test (FOBT) reduces CRC mortality. These trials have different designs, especially concerning FOBT frequency and duration, as well as the length of follow-up after stopping FOBT campaigns.

Aims: To review the effectiveness of screening for CRC with FOBT, to consider the reduction in mortality during or after screening or to identify factors associated with a significant mortality reduction.

Objective: The immunosuppressive properties of azathioprine (AZA) are mediated by intracellular metabolism of 6-MP into its active metabolites 6-thiguanine nucleotide (6-TGN) and 6-methylmercaptopurine (6-MMP). The aims of this study were to correlate red blood cell (RBC) 6-TGN and hematological parameters and their change in adult patients with Crohn's disease (CD) treated by AZA and to determine independent factors enabling determination of RBC 6-TGN.

Methods: RBC 6-TGN concentration was determined with high performance liquid chromography (HLPC) performed on 74 hepa-rinized blood samples from 32 patients.

Background And Aims: Following ileocolonic resection for Crohn's disease (CD), early endoscopic recurrence predicts recurrence of symptoms. The aim of the study was to compare ileocolonoscopy and wireless capsule endoscopy (WCE) for the detection of postoperative recurrence in CD.

Methods: WCE and ileocolonoscopy were performed within six months following surgery in 32 prospectively enrolled patients.
