Background: Most trauma occurs among young male subjects in Qatar. We examined the predictive values of the delta shock index (DSI), defined as the change in the shock index (SI) value from the scene to the initial reading in the emergency unit (, subtracting the calculated SI at admission from SI at the scene), at a Level 1 trauma center.

Aim: To explore whether high DSI is associated with severe injuries, more interventions, and worse outcomes [, blood transfusion, exploratory laparotomy, ventilator-associated pneumonia, hospital length of stay (HLOS), and in-hospital mortality] in trauma patients.

Article Synopsis
  • The study evaluated a clinical decision tool (CDT) for assessing thoracolumbar fractures in trauma patients, aiming to potentially replace traditional imaging methods to speed up the clearance of the TL spine.
  • Conducted on alert trauma patients with suspected injuries, the CDT tested for criteria like pain absence and movement capability to determine fracture presence.
  • Results showed the CDT had a high sensitivity (99.38%) for identifying the absence of fractures, but lower specificity (9.1%), indicating it was better at ruling out fractures than confirming them.

In October 2018, the United Network for Organ Sharing changed their heart allocation criteria to prioritize patients on temporary mechanical circulatory support. This study assesses outcomes of patients bridged to orthotopic heart transplantation (OHT) with venoarterial extracorporeal membrane oxygenation (VA ECMO) since this change. We conducted a retrospective single-center study of adults (≥18 years) supported with VA ECMO at the time of OHT (October 1, 2018, to December 31, 2021).

Article Synopsis
  • - Cardiac tumors are rare, with metastatic tumors being significantly more common than primary ones, and multimodal imaging is essential for evaluating their characteristics and effects on the heart.
  • - A case is presented involving a patient with a large mass in the left ventricle that caused unstable ventricular tachycardia, highlighting the use of various imaging techniques to assess the mass.
  • - It’s important to consider metastatic melanoma when diagnosing cardiac masses, and patients with such masses should be questioned about any previous skin cancers, as effective imaging aids in treatment planning and monitoring.

  • Community pharmacists play a crucial role in implementing pharmacogenomics by educating other healthcare professionals about its significance and promoting its use in practice in the UAE.
  • A survey conducted among 586 community pharmacists revealed that they had an average knowledge score of 75.1% and an attitude score of 67.5% regarding pharmacogenomics, with certain groups showing better scores based on experience, training, and pharmacy type.
  • The study highlights the importance of targeted training for pharmacists to enhance their understanding and attitudes towards pharmacogenomics, as it could impact patient care and medication management.

Cardiac allografts suffer diastolic dysfunction early post-heart transplantation (HTx) due to ischemic injury, however the natural course of diastology recovery post HTx remains unknown (Tallaj et al., 2007 [1]). We retrospectively reviewed 60 adult HTx patients between 2015 and 2021 at a single site.

Introduction: Diuretics are the mainstay of maintaining and restoring euvolemia in the management of heart failure. Loop diuretics are often preferred, however, combination diuretic therapy (CDT) with a thiazide diuretic is often used to overcome diuretic resistance and increase diuretic effect. We performed an analysis of the GUIDE-IT study to assess all-cause mortality and time to first hospitalizations in patients necessitating CDT.

This is an interim analysis of the Beta-blocker (Propranolol) use in traumatic brain injury (TBI) based on the high-sensitive troponin status (BBTBBT) study. The BBTBBT is an ongoing double-blind placebo-controlled randomized clinical trial with a target sample size of 771 patients with TBI. We sought, after attaining 50% of the sample size, to explore the impact of early administration of beta-blockers (BBs) on the adrenergic surge, pro-inflammatory cytokines, and the TBI biomarkers linked to the status of high-sensitivity troponin T (HsTnT).

Cardiogenic shock (CS) is a severe complication of peripartum cardiomyopathy (PPCM). Patients with deteriorating CS often require temporary mechanical circulatory support. In PPCM, this can be used as a bridge to postpartum recovery or bridge to decision.

Cardiac sympathetic denervation (CSD) is a surgical procedure increasingly used for managing ventricular arrhythmia refractory to conventional medical therapy. Long-term outcomes of CSD in patients with systolic heart failure has not been well studied. This observational study aimed to evaluate the medical co-morbidities and outcomes of patients with systolic heart failure who underwent CSD performed as treatment for ventricular arrhythmia refractory to conventional therapy.

Background: The use of Intra-aortic Balloon Pump (IABP) and Impella devices as a bridge to heart transplantation (HTx) has increased significantly in recent times. This study aimed to create and validate an explainable machine learning (ML) model that can predict the failure of status two listings and identify the clinical features that significantly impact this outcome.

Methods: We used the UNOS registry database to identify HTx candidates listed as UNOS Status 2 between 2018 and 2022 and supported with either Impella (5.

Background: The percentage of brain tumor incidence exceeds 50% in the geriatric population admitted at Khoula Hospital (a tertiary care hospital in Oman) as compared to the younger population, furthermore, geriatric patients impose a higher cost of healthcare in general. Therefore, geriatric tumor care is causing a significant burden on the healthcare service in Oman. For this reason, we have developed this study to identify the cost of care for this group.

The left ventricular assist device (LVAD) is a mechanical circulatory support device that supports the heart failure patient as a bridge to transplant (BTT) or as a destination therapy for those who have other medical comorbidities or complications that disqualify them from meeting transplant criteria. In patients with severe heart failure, LVAD use has extended survival and improved signs and symptoms of cardiac congestion and low cardiac output, such as dyspnea, fatigue, and exercise intolerance. However, these devices are associated with specific hematologic and thrombotic complications.

A 14-year-old male with no significant medical history presented with intermittent palpitations for 2-3 months that occurred at rest and were associated with light-headedness. Electrocardiogram in clinic showed sinus arrhythmia with early repolarization and no ischemic changes. The echocardiogram showed normal cardiac structure and function, however, there was a concern for possible anomalous origin of the left coronary artery.

Article Synopsis
  • The study reviews how cognitive frailty assessment impacts outcomes for patients receiving left ventricular assist devices (LVAD), highlighting variability in findings and assessment methods.
  • There were 12 studies involving over 16,000 patients, primarily using the Montreal Cognitive Assessment, with mixed results on outcomes like length of hospital stay, readmissions, and mortality.
  • The authors suggest that cognitive frailty may worsen post-LVAD outcomes but call for better standardization in reporting and further research to clarify its prognostic implications.
