Congenital Long QT Syndrome Unmasked by Albuterol in an Adolescent with Asthma.

Background: Patients with congenital long QT syndrome (LQTS) are prone to ventricular dysrhythmia but may be initially asymptomatic with a normal QTc interval on resting electrocardiogram (ECG). Albuterol is listed as a medication that poses a "special risk" to patients with congenital LQTS, but its effects have been rarely described. We present a case of previously unknown, asymptomatic congenital LQTS unmasked by albuterol in an adolescent with asthma.

Pancreatitis, panniculitis and polyarthralgia syndrome: A rare complication of pancreatic pathology.

Pancreatitis, panniculitis, and polyarthralgia (PPP) syndrome is a rare complication of chronic pancreatitis and occurs due to leakage of pancreatic enzymes in the systemic vasculature. This enzyme leakage leads to multiple manifestations such as polyarthralgia, panniculitis, and bone necrosis due to tissue autodigestion.The inciting pancreatic pathology may be masked, and the presentation may be due to one of the systemic consequences of enzyme leakage, which can present as a diagnostic challenge for clinicians.

Reconstruction of Tibial Plateau Fracture Malunion in the Setting of a Large Cartilage Defect in an Adolescent: A Case Report.

Case: An 18-year-old adolescent boy presented with knee pain and stiffness secondary to tibial plateau valgus malunion and osteochondral defect, 8 months after initial injury/fixation. We opted for a novel technique that reconstructs the convex lateral tibial plateau by using osteotomy and an osteochondral autograft harvested from the lateral aspect of the ipsilateral femoral condyle.

Conclusion: The reported novel reconstruction technique is inexpensive, achievable with routine techniques, and demonstrated a favorable short-term outcome.

Tuberculous Extensor Tenosynovitis Presenting as Huge Mass on the Dorsum of the Hand.

Infectious tuberculous tenosynovitis (TS) of the extensor tendons of the wrist is an exceptional location of musculoskeletal tuberculosis. We present a case of tuberculous extensor TS in a 52-year-old diabetic male patient presenting as a huge mass on the dorsum of the hand, in the absence of other pulmonary or extrapulmonary manifestation of tuberculosis. This report increases physicians' vigilance when dealing with patients with risk factors of tuberculosis, allowing early diagnosis and optimal treatment.

[Burnout and psychosocial risk factors in the career of flight attendants: Descriptive study of a population of Lebanese flight attendants].

Objective: The aim of our study was to investigate the influence of demographic factors (gender and age), work related factors (the position and years of practice) and psychosocial risk factors (cognitive demands, demands to hide emotions, work-family conflict, offensive behaviors…) on the development of burnout among flight attendants, and the nature of psychosocial factors to which they are exposed.

Methods: The sample was divided into two groups: 67 flight attendants and 67 persons in various jobs. Our sampling units were informed and consented to fill in two questionnaires: the Copenhagen burnout inventory (CBI) and the Copenhagen Psychosocial Questionnaire (COPSOQ II).

Can dual-headed 18F-FDG SPET imaging reliably supersede PET in clinical oncology? A comparative study in lung and gastrointestinal tract cancer.

In this study, we prospectively compared the sensitivity of PET and planar SPET (collimated gamma camera) 18F-FDG imaging in patients with lung and gastrointestinal tract cancer and analysed their respective impact on patient management. Twenty-eight patients with lung cancer and 14 with gastro-intestinal tract tumours were scanned on the same day with a PET and a collimated planar SPET gamma camera. The planar SPET procedure consisted of whole-body planar views and a tomographic acquisition centred over the torso or the abdomen, with the total imaging time within the same range as the whole-body PET procedure.

Superior vena cava obstruction after extracorporeal membrane oxygenation.

Of 60 neonates who survived extracorporeal membrane oxygenation (ECMO) in our institution between June 1992 and March 1994, seven had either complete or partial superior vena cava (SVC) obstruction. When the patients with SVC obstruction were compared with those who had an echocardiogram after ECMO, no predisposing factors for the development of SVC thrombus could be found. Our data show that SVC thrombus may be a significant complication after ECMO.

[Can one determine the age at which epiphysiodesis should be performed in the treatment of genu valgum in the adolescent?].

The authors describe a simplified method for the assessment of the correct time at which tibial epiphyseal arrest may be performed to correct genu valgum in adolescents. The calculation is based on measurements of the length of the tibia, the length of the upper tibial plate and the intermalleolar distance. These measurements make it possible to establish an index of prediction of the amount of growth necessary to bring about correction.

[Treatment of bimalleolar fractures by centromedullary nailing of the fibula].

An osteosynthesis technique involving nailing of the fibula was used to treat 106 bimalleolar fractures. Details of the technique used are described and results analyzed with respect to clinical course and radiologic findings. A generally favorable outcome was obtained.
