Article Synopsis
  • Ventricular arrhythmias (VAs) are uncommon but can occur after electrical cardioversion (ECV) for atrial fibrillation (AF), with an incidence of about 0.2% based on a study of nearly 12,000 ECV procedures.
  • The study involved 23 patients who experienced VAs within 10 days of ECV, with most cases developing 28.5 hours after the procedure, and a significant number had underlying health issues like congestive heart failure.
  • The findings suggest that while VAs post-ECV are rare, they can be life-threatening, especially in patients with certain risk factors, indicating that careful monitoring is essential for these individuals.

On October 7, 2023, somewhere around 1,500-3,000 terrorists invaded southern Israel killing 1,200 people, injuring 1,455, and taking 239 as hostages resulting in the largest mass-casualty event (MCE) in the country's history. Most of the victims were civilians who suffered from complex injuries including high-velocity gunshot wounds, blast injuries from rocket-propelled grenades, and burns. Many would later require complex surgeries by all disciplines including general surgeons, vascular surgeons, orthopedists, neurosurgeons, cardiothoracic surgeons, otolaryngologists, oral maxillofacial surgeons, and plastic surgeons.

Introduction: Measuring elevated post-void residual volume is important for diagnosing urinary outflow tract obstruction and cauda equina syndrome. Catheter placement is exact but painful, invasive, and may cause infection, whereas an ultrasound is accurate, painless, and safe.

Aim: The purpose of this single-center study is to evaluate the accuracy of a module for artificial-intelligence (AI)-based fully automated bladder volume (BV) prospective measurement using two-dimensional ultrasound images, as compared with manual measurement by expert sonographers.

Article Synopsis
  • - The text discusses Calcium Release Deficiency Syndrome (CRDS), a serious genetic heart condition that can cause sudden cardiac arrest without clear reasons and is not detectable through standard tests.
  • - The study aimed to analyze electrocardiogram (ECG) responses after brief periods of fast heart rates followed by pauses in order to develop a diagnostic test for CRDS.
  • - Findings showed that patients with CRDS had a significantly greater change in T-wave amplitude on their ECG after a pause compared to control groups, indicating a potential diagnostic marker for this syndrome.

Article Synopsis
  • Point-of-care ultrasound is widely used by physicians to aid in diagnostics, particularly in assessing left-ventricular ejection fraction (LVEF) in patients.
  • A study involving trained medical students used an AI tool to evaluate LVEF in hospitalized patients, finding that reduced LVEF was linked to higher rates of mortality and readmission for cardiovascular issues.
  • The results suggest that even novice users, like medical students, can leverage AI in ultrasound assessments to predict patient outcomes and identify those at higher risk for complications.

We sought to prospectively investigate the accuracy of an artificial intelligence (AI)-based tool for left ventricular ejection fraction (LVEF) assessment using a hand-held ultrasound device (HUD) in COVID-19 patients and to examine whether reduced LVEF predicts the composite endpoint of in-hospital death, advanced ventilatory support, shock, myocardial injury, and acute decompensated heart failure. COVID-19 patients were evaluated with a real-time LVEF assessment using an HUD equipped with an AI-based tool vs. assessment by a blinded fellowship-trained echocardiographer.

Background: Enterococcal Infective Endocarditis (EIE) is usually treated with the combination of penicillin/ampicillin with gentamicin or ampicillin with ceftriaxone. To enable prolonged outpatient treatment, a combination of benzylpenicillin and ceftriaxone has been suggested. This study aimed to describe the incidence and characteristics of EIE and to determine the outcome of EIE cases treated with benzylpenicillin and ceftriaxone.

Strenuous physical exercise causes a massive elevation in the concentration of circulating cell-free DNA (cfDNA), which correlates with effort intensity and duration. The cellular sources and physiological drivers of this phenomenon are unknown. Using methylation patterns of cfDNA and associated histones, we show that cfDNA in exercise originates mostly in extramedullary polymorphonuclear neutrophils.

The role of point-of-care ultrasound (POCUS) in patient management has been established in recent years as an important tool. It is increasingly used by multiple medical disciplines in numerous clinical settings, for different applications and diagnostic purposes and in the guidance of procedures. The introduction of small-sized and inexpensive hand-held ultrasound devices (HUDs) has addressed some of the POCUS-related challenges and has thus extended POCUS' applicability.

Introduction: Hypoalbuminemia is common in acute and chronic diseases. It has been proposed as a potential biomarker of frailty, which itself is associated with worse outcomes. However, data regarding the level of hypoalbuminemia and its prognosis in contemporary intensive coronary care unit (ICCU) patients is scarce.

Objectives: Incorporating artificial intelligence (AI) into echocardiography operated by clinicians working in the emergency department to accurately assess left-ventricular ejection fraction (LVEF) may lead to better diagnostic decisions. This randomized controlled pilot study aimed to evaluate AI use as a didactic tool to improve noncardiologist clinicians' assessment of LVEF from the apical 4-chamber (A4ch) view.

Methods: This prospective randomized controlled pilot study tested the feasibility and acceptability of the incorporation of AI as a didactic tool by comparing the ability of 16 clinicians who work in the emergency department to assess LVEF before and after the introduction of an AI-based ultrasound application.
