Background: The registered nurse (RN) workforce experienced critical pre-pandemic and pandemic shortages of labor in some areas in the United States. People living in these health professional shortage areas (HPSAs) may have less access to health services. The Bureau of Health Workforce within the Health Resources and Services Administration administers Nurse Corps scholarship and loan repayment programs to increase healthcare access by increasing the supply and distribution of RNs, nurse practitioners, and nurse faculty to HPSAs.

To determine the incidence of thromboembolism in relation to thoracotomy, 77 patients undergoing pulmonary resection were prospectively studied up to 30 days postoperatively for deep venous thrombosis and pulmonary embolism. Overall, 20 of 77 patients (26%) had thromboembolic events during their hospitalization. Four deep venous thromboses and 1 pulmonary embolism were detected in 5 of 77 patients preoperatively for an incidence of 6%.

Because the left upper lobe bronchus overlies the left pulmonary artery (PA), T2-3 lesions, N0-1 disease, or rarely inflammation may involve this vessel, necessitating lobectomy with partial PA resection or pneumonectomy with sacrifice of the lower lobe. In 486 operations performed for left upper lobe lesions between 1966 and 1992 (wedge, 111; segmentectomy, 131; lobectomy, 155; pneumonectomy, 89), isolated PA encroachment was caused by bronchogenic carcinoma (32), invasive aspergillosis (2), or organized pneumonitis (1) and occurred in 9% (32/360) of malignant left upper lobe tumors and 2% (3/126) of benign lesions. Initially (1966 through 1979), PA involvement was the indication for 30% (18/60) of left pneumonectomies.

Despite continuing improvement in myocardial protection and surgical technique, the repair of complex congenital heart lesions can result in cardiopulmonary compromise refractory to conventional therapy. In a 29-month period, 24 patients (aged 14 hours to 6 years) were treated with extracorporeal membrane oxygenation (ECMO) 28 times for profound cardiopulmonary failure. Four patients required ECMO after each of two cardiopulmonary bypass procedures.

A patient underwent resection of a superior sulcus tumor after an extensive workup was nondiagnostic. Postoperatively, primary pulmonary cryptococcosis was diagnosed. Management of this patient is presented along with a review of the literature.

The objective of this study was to prospectively assess the value of combining clinical assessment and noninvasive testing in predicting the spectrum of carotid bifurcation pathology, as subsequently proven by arteriography, in order to determine the safety and accuracy of performing carotid endarterectomy without angiography. A panel of eight specialists representing vascular surgery, neurology, and neurosurgery were presented with the history, physical findings, and noninvasive test results (GEE-OPG and duplex scan) of 85 patients. They were asked to make an anatomic prediction of the status of each carotid artery (170 arteries) as to whether the bifurcation was normal, ulcerated, had a hemodynamically significant stenosis, or was occluded.

This experiment evaluated the effect of aspirin (ASA) on the patency and development of intimal hyperplasia in autologous, small caliber, arterial end-to-side anastomosis. Twenty-eight adult female rabbits had their distal left external iliac artery transposed and anastomosed to the right proximal external iliac artery. Fourteen rabbits received ASA 40 mg by gavage twice in the 24 hours preceding the operation and 5 times a week postoperatively (experimental) and 14 rabbits received no ASA therapy (control).

Seventy-two patients who underwent orthotopic liver transplantation (OLT) were studied to identify perioperative variables that would predict survival and intraoperative blood loss. Survival and intraoperative blood loss were not affected by encephalopathy, length of donor liver ischemia, or any of the preoperative laboratory values studied. Survival was significantly decreased in patients requiring postoperative dialysis (41%) and in patients who had severe rejection requiring retransplantation (33%).

Previous studies have demonstrated that standard knitted and woven fabric grafts are forever dependent on the suture material for anastomotic tensile strength. Clinical experience with polytetrafluoroethylene (PTFE) and double velour knitted grafts have shown that there is extensive fibrous capsular bonding between the graft and the surrounding tissues. This would lead to increased anastomotic tensile strength.

Percutaneous intra-arterial infusion of fibrinolytic agents has emerged as an alternative to embolectomy in selected patients with acute arterial occlusions. The combination of fibrinolytic therapy and embolectomy may be superior to either modality alone. This experiment was designed to determine safety and efficacy of intraoperative fibrinolytic therapy as an adjunct to catheter embolectomy.

We have developed a technique to render knitted Dacron prostheses totally impervious to blood leak by impregnating the cloth with collagen. These grafts were prepared by the manufacturer, sterilized, and have an indefinite shelf life. The objective of this study was to determine whether collagen impregnation has a deleterious effect on surface thrombogenicity and graft healing.

From May 1964 to June 1983, 36 carotid-subclavian bypasses were done in 36 patients who had symptomatic lesions at the origin of the common carotid and/or subclavian arteries at the Center for Health Sciences of the University of California, Los Angeles. Ages ranged from 28 to 82 years (mean, 58 years). Eighteen bypasses were done with prosthetic grafts, 13 done with autogenous vein, and five were transpositions with primary anastomosis of the subclavian and carotid arteries.
