Introduction And Importance: This case highlights the importance of echocardiography in diagnosing pulmonary embolism and the need for careful timing of preoperative transesophageal echocardiogram in patients with potentially detachable intracardiac masses as a tumor detachment during TEE led to a life-threatening pulmonary embolism.
Case Presentation: A 30-year-old man with a history of treated pulmonary embolism had progressive dyspnea. Tests revealed a mass in the right atrium and a new pulmonary embolism.
Myelofibrosis (MF) is a myeloproliferative neoplasm (MPN) with a poor prognosis, and allogeneic hematopoietic stem cell transplantation (allo-HSCT) is the only treatment with curative potential. Ruxolitinib, a JAK1/2 inhibitor, has shown promising results in improving patients' symptoms, overall survival, and quality of life, and can be used as a bridging therapy to HSCT that increases the proportion of transplantable patients. However, the effect of this and similar drugs on HSCT outcomes is unknown, and the reports on their efficacy and safety in the peri-transplantation period vary widely in the published literature.
