Background: The aim of this study was to evaluate the relationship between antiinflammatory cytokine interleukin (IL)-10 production and perioperative lactate concentrations and their impact on postoperative outcomes in neonates undergoing the arterial switch operation (ASO).
Methods: Between August 2010 and August 2012, 80 neonates with transposition of the great arteries (TGA) were enrolled. Serum IL-10 levels were measured immediately before and after cardiopulmonary bypass (CPB) on the first, third, and seventh days.
The results of perinatal management and surgical treatment of 30 newborn babies, owing antenatally diagnosed isolated aortal coarctation (AC) and 40 newborn babies, in whom this diagnosis was not established antenatally, were analyzed. Application of antenatal diagnosis and optimization of the patients' perinatal management algorithm have permitted to reduce the treatment duration in the intensive therapy division of cardiosurgical department. In newborn babies, to whom the diagnosis of AC was established antenatally, a trustworthy reduced duration of postoperative pulmonary artificial ventilation was revealed in comparison with such in patients, in whom this diagnosis was not established antenatally.

Comparative investigation was done on the antibodies titre to Hsp60 in the blood serum of a newborn babies, suffering critical inborn heart failure, to whom autologous cord blood or the donor's blood components was transfused, in early and remote postoperative period. In early postoperative period the lowering of the antibodies titre to Hsp60 in the blood serum was observed in comparison with them preoperatively, in a late period (in 2 yrs) in all the blood serum samples investigated antibodies to Hsp60 were not revealed.

Methods: We compared perioperative hematocrit (Hct), fetal hemoglobin (HbF), p(50)O(2), pH, pCO(2), serum lactate, duration of mechanical ventilation and intensive care unit (ICU) length of stay in neonates undergoing open-heart surgery using autologous umbilical cord blood (Group I, N=45) or donor blood components (Group II, N=65). The groups were similar with respect to diagnosis, weight, type of procedure, duration of CPB and duration of myocardial ischemia.
Introduction: For the evaluation of the congestive heart failure (CHF) in rat models, the use of special equipment for echocardiography for dynamic evaluation is suggested. The optimal doxorubicin dose for CHF induction has still not been established.
Aims: The aims of our study was to find a reliable doxorubicin CHF rat model using a general ultrasound (US) equipment for in vivo ultrasound examination of the systemic circulation, to establish the optimal doxorubicin dose, and to assess the feasibility of US guided administration of the drug.
We investigated the serum levels ofproinflammatory and antiinflammatory cytokines (TNF-alpha, IL-1beta, IL-6, IL-8, IL-10) in newborns with transposition of the great arteries to whom during the defect correction the autologous umbilical cord blood and blood components were administered before the surgery and at the 1st, 3rd, 7th day after the surgery. We found that in the group of newborns to whom during the operation the blood components were used, the levels ofpro-inflammatory interleukins were high before surgery and at the Ist, 3rd and 7th day after it, but IL-10 was reduced. During the postoperative period, the newborns of this group had imbalance in the system cytokine, accompanied by clinical complications such as hyperthermia and pulmonary complications.

A global tendency of enhancement of a prenatal diagnosis establishment rate in critical inborn heart failures demands to overestimate the existing surgical strategy. Taking into account a present sufficient compensation of a newborn babies state, suffering main vessels transposition, in first hours of the patients life and existing possibility to approve a surgical tactics before the patient's birth after the prenatal diagnosis establishment, we have proposed an innovative approach, consisting of conduction of the arterial switching operation without preliminary performance of the balloon atrioseptostomy and preoperative medicinal therapy. The efficacy and security of a new approach were analyzed.
