Aims: Patients with heart failure with preserved ejection fraction represent half of the heart failure patients nowadays, an at least steady trend due to the aging of the population. We investigated whether the parameters obtained from cardiopulmonary exercise testing (CPET) correlated with the prognosis of these patients. This prospective observational cohort study assesses the relationship between the CPET parameters peakVO and VE/VCO slope and the number of heart failure hospitalizations or cardiovascular death of these patients.

Article Synopsis
  • A new method called the subcostal en-face view of the tricuspid valve (TV) was developed to visually assess the position of right ventricular (RV) leads in patients with cardiac implantable electronic devices (CIED).
  • A study involving 176 patients found that 64% had their RV lead position successfully identified using this method, with 27% showing at least moderate tricuspid regurgitation (TR).
  • However, the position of the RV lead did not correlate with the severity of TR in these patients.

Article Synopsis
  • The relationship between the body and its environment influences vital host-microbiome interactions that affect how diseases differ between sexes, including susceptibility, symptoms, and treatment results.
  • Factors like hormones, immune responses, and genetic differences contribute to these discrepancies and interact throughout a person's life, while the gut microbiome plays a significant role in various body systems.
  • The paper reviews current knowledge on how sex hormones, gut bacteria, and overall health relate to cardiovascular disease differences between genders, highlighting research gaps and the need for a more integrated approach in future studies.

White cord syndrome is an extremely rare type of cervical spinal cord ischemia characterized by sudden neurological deterioration following surgical spinal decompression. The underlying cause is believed to be immediate relief from chronic compression on the spinal cord, triggering an inflammatory response known as ischemic reperfusion injury. A 49-year-old male presented in the office with neck pain and chronic symptoms of progressive cervical myelopathy: clumsiness, gait instability, and dropping things.

Iron-containing metal-organic frameworks are promising Fenton catalysts. However, the absence of additional modifiers has proven difficult due to the low reaction rates and the inability to manipulate the catalysts. We hypothesize that the production of iron oxide NPs in the presence of a metal-organic framework will increase the rate of the Fenton reaction and lead to the production of particles that can be magnetically manipulated without changing the structure of the components.

Introduction: There is substantial debate on the best method to reverse factor Xa-inhibitors in patients following traumatic brain injury (TBI). Prothrombin complex concentrates (PCC) have been used for this indication but their role has been questioned. This study reported failure rates with PCC in patients following TBI and as a secondary objective, compared 4-factor (4 F-PCC) and activated PCC (APCC).

During production of radiopharmaceuticals, the radiation situation in cyclotron pit is an important parameter, which is being monitored to ensure fulfilment of the limits and conditions of safe operation. The neutron flux in the structural components of the accelerator is also an important parameter, because the secondary neutrons are responsible for activation of cyclotron structural components and may even affect structural changes in it. This paper aims to characterize the neutron field in inner positions of medical accelerator IBA 18/9 by activation detectors and by means of scintillation spectrometry.

Adequate grading of mitral regurgitation (MR) in patients with mitral valve prolapse (MVP) in the presence of mid-late systolic jets can represent a major challenge. In this entity, jets are commonly overestimated by echocardiography. Correct quantification is crucial and highly relevant for the further management and prognosis of these oftentimes young patients.

Aims: Heart failure with preserved ejection fraction (HFpEF) is one of the most rapidly growing cardiovascular health burden worldwide, but there is still a lack of understanding about the HFpEF pathophysiology. The nitric oxide (NO) signalling pathway has been identified as a potential key element. The aim of our study was to investigate markers of NO metabolism [l-arginine (l-Arg), homoarginine (hArg), and asymmetric and symmetric dimethylarginine (ADMA and SDMA)], additional biomarkers [N-terminal pro-B-type natriuretic peptide (NT-proBNP), endothelin-1 (ET-1), mid-regional pro-adrenomedullin (MR-proADM), copeptin, and high-sensitivity C-reactive protein (hsCRP)], and the endothelial function in an integrated approach focusing on associations with clinical characteristics in patients with HFpEF.

Objectives: The Distal Motor Function (DMF) sub-score of the NIH Stroke Scale (NIHSS) was measured in the NINDS rt-PA Stroke Trials but is currently not included in the NIHSS. The correlation of DMF with the NIHSS Motor Arm Function (MAF) sub-score, the effect of IV tPA treatment on DMF, and whether adding DMF changes the utility of the NIHSS have not been analyzed.

Materials And Methods: MAF and DMF sub-scores were retrieved from the original NINDS rt-PA Stroke Trials for both sides of the body at baseline, 2 hours, 24 hours, 7-10 days, and 3 months after IV tPA treatment.

Aims: Exercise intolerance is the leading manifestation of heart failure with preserved or mid-range ejection fraction (HFpEF or HFmrEF), and left atrial (LA) function might contribute to modulating left ventricular filling and pulmonary venous pressures. We aim to assess the association between LA function and maximal exercise capacity in patients with HFpEF or HFmrEF.

Methods And Results: Sixty-five patients, prospectively enrolled in the German HFpEF Registry, were analysed.

Heart failure (HF), with steadily increasing incidence rates and mortality in an ageing population, represents a major challenge. Evidence suggests that more than half of all patients with a diagnosis of HF suffer from HF with preserved ejection fraction (HFpEF). Emerging novel biomarkers to improve and potentially guide the treatment of HFpEF are the subject of discussion.

Background/objective: Desmopressin (DDAVP) has been suggested for antiplatelet medication reversal in patients with traumatic brain injury (TBI) but there are limited data describing its effect on clinical outcomes. The purpose of this study was to evaluate the effect of DDAVP on hematoma expansion and thrombosis in patients with TBI who were prescribed pre-injury antiplatelet medications.

Methods: Consecutive adult patients who were admitted to our level I trauma center and prescribed pre-injury antiplatelet medications between July, 2012, and May, 2018, were retrospectively identified.

Stress ulcer prophylaxis (SUP) with acid-suppressive drug therapy is widely utilized in critically ill patients following neurologic injury for the prevention of clinically important stress-related gastrointestinal bleeding (CIB). Data supporting SUP, however, largely originates from studies conducted during an era where practices were vastly different than what is considered routine by today's standard. This is particularly true in neurocritical care patients.

Background: Traumatic brain injury remains a challenging and complicated disease process to care for, despite the advance of technology used to monitor and guide treatment. Currently, the mainstay of treatment is aimed at limiting secondary brain injury, with the help of multiple specialties in a critical care setting. Prognosis after TBI is often even more challenging than the treatment itself, although there are various exam and imaging findings that are associated with poor outcome.
