Rates of cardiovascular mortality and morbidity in Russia have been among the highest in Europe. A comprehensive health system improvement program targeting better diagnosis and control of hypertension was undertaken in the Yaroslavl Region of Russia. This initiative was a joint program between clinicians, the Department of Health and Pharmacy of the Yaroslavl Region, and Novartis Pharma LLC.

For the last 26 years operations have been performed on 528 patients for carcinoma of the cardiac portion of the stomach. In 392 of them extensive, combined and extensive-combined operations were fulfilled. These operations are considered by the authors to be referred to a group of conventionally radical operations.

Among 438 patients subjected to radical surgery for cancer of the gastric cardia in 319 cases extensive, combined and extensive-combined surgical procedures were accomplished, their characteristics being described. The regular feature of cancer of this localization is its lymphogenic spread, and this fact together with esophageal involvement allows a consideration that for cancer of the cardia, in any stage but 1, all resections, as a rule, should be extensive, combined or both.
